A Systematic Review of Outcomes after Dacryocystorhinostomy in Adults

Abstract: DCR is an effective and safe method for the treatment of nasolacrimal obstruction. Meta-analysis of outcomes was not feasible because of the heterogenous patient groups and outcome measures used. Nonetheless, the literature provides considerable levels 3 and 4 evidence to support DCR surgery in adults. Outcomes after EN-DCR and EX-DCR were comparable. The failure rate for LA-DCR was higher.

“…The results are in correlation with studies done by previous authors & support their results (24)(25)(26)(27) (28).The striking predilection for females can be explained by the narrower lumen of the bony naso-lacrimal canal. It is also possible that endocrine factors may be playing a role in the aetiology of chronic dacryocystitis.…”

“…Canalicular silicone intubation at the end is widely favoured and used to prevent fibrous closure and sealing of the edges of the sac during healing (Yagci et al 1999;Caversaccio & Hausler 2006;Liang & Lane 2013). Studies have shown that the endoscopic DCR success percentages vary widely the last decades between 81% and as high as 97% in experienced hands (Tarbet & Custer 1995;Sprekelsen & Barberan 1996;Hartikainen et al 1998;Woog et al 2001;Durvasula & Gatland 2004;Tsirbas et al 2004;Ben Simon et al 2005;Leong et al 2010;Roithmann et al 2012). The endoscopic DCR has become a well-established technique the last few years, with a safety profile and efficacy equivalent if not better than to the traditional external approach (Hartikainen et al 1998).…”

“…2 Dacryocystorhinostomy (DCR) is an effective and safe method for the treatment of NLD obstruction. 3 DCR for the treatment of NLD obstruction was first described via an external approach by Toti in 1904. 4 The perceived disadvantages of the external approach DCR include the risk of cutaneous scar and lengthy surgery with significant blood loss.…”
