A Systematic Review and Meta-Analysis of Diagnostic Performance of MRI for Evaluation of Acute Appendicitis

Abstract: MRI has a high accuracy for the diagnosis of acute appendicitis, for a wide range of patients, and may be acceptable for use as a first-line diagnostic test.

“…It has been proposed to be used in pregnant patients with abdominal pain when US is inconclusive [33]. Recently, a systematic review and meta-analysis of diagnostic performance of MRI for evaluation of acute appendicitis was published [34]. A total of 30 studies that comprised 2665 patients were reviewed.…”

“…Its high soft‐tissue contrast resolution, lack of ionizing radiation, and reduced operator dependency are strong advantages in children. Despite a substantial number of studies on the use of MRI in the evaluation of acute appendicitis in children and adolescents, there are only a few published systematic reviews and meta‐analyses . Hence, we conducted a systematic review and meta‐analysis with a larger number of included studies and using hierarchical models to evaluate the diagnostic performance of MRI and the negative appendectomy rate for suspected appendicitis in pediatric patients.…”

“…However, due to the potential risk of malignancy [15, 30], serial US [31] and magnetic resonance imaging (MRI) [32, 33] have recently been suggested as alternatives to CT. A meta-analysis proposed MRI as an optional first line imaging modality for suspected appendicitis in children [32]. However, availability, cost, and the possible need for sedation in young children make the practical use of MRI called into question.…”
