A systematic review and meta-analysis of 18F-fluoro-d-deoxyglucose positron emission tomography interpretation methods in vascular graft and endograft infection

Abstract: Objective: Vascular graft and endograft infection (VGEI) has high morbidity and mortality rates. Diagnosis is complicated because symptoms vary and can be nonspecific. A meta-analysis identified 18 F-fluoro-D-deoxyglucose positron emission tomography-computed tomography ( 18 F-FDG PET/CT) as the most valuable tool for diagnosis of VGEI and favorable to computed tomography as the current standard. However, the availability and varied use of several interpretation methods, without consensus on which interpretati… Show more

“…LVAD infections are diagnostically challenging and difficult to treat, as source culture and source control (device removal) may not be easily possible. Similarly, it may also have a key role in diagnosing vascular graft infection (VGI)—both native vessel and endoprosthesis—for which it has high pooled sensitivity (>90%) and moderate-to-high pooled specificity (59–81%) [ 72 , 73 , 74 , 75 ].…”

“…Also, it detects radiolabeled leukocyte uptake by white blood cell (WBC) scintigraphy based on the capacity of a radiolabeled granulocyte to reach the infection site and to accumulate in infectious tissues. WBC scintigraphy together with single-photon emission computed tomography (SPECT/CT) seems to increase diagnostic power [21,[84][85][86][87][88][89][90][91][92][93][94][95][96][97][98][99][100][101][102][103].…”

The importance of MAGIC criteria, which are also recommended in the current guidelines [22], lies in the provided definition of classic and definite cases that can be managed with high sensitivity and specificity.…”

“…Although there is a tendency from open surgery to more endovascular procedures, vascular graft and endograft infection (VGEI) still occurs in approximately up to 6% of all vascular graft replacements [1,2]. Early postoperative infections mainly occur due to intraoperative contamination or a direct progression of a wound infection to the graft, while VGEI occurring a long time after surgery is considered to be a result of the reactivation of a previously undetected infection of the graft or seeding of the graft due to bacteraemia [3].…”
