A systematic review and evidence synthesis of non-medical triage, self-referral and direct access services for patients with musculoskeletal pain

Abstract: Introduction The demand for musculoskeletal (MSK) care is rising, and is a growing challenge for general practice. Direct access to physiotherapy and other healthcare services may offer appropriate care for MSK pain patients but there is uncertainty regarding the effectiveness or efficiency of this approach in practice. This study aimed to review the evidence regarding characteristics, outcomes, barriers and facilitators of MSK triage and direct access services. Methods A comprehensive search of eight database… Show more

“…Lower disease prevalence in primary care results in lower yields from diagnostic strategies and guidelines have increasingly called for more selective imaging. 30 31 Additionally, investment in readily accessible and evidenced community MSK 'interface' triage services, [32][33][34][35] recommended in national MSK transformation strategy, 36 calls into question the need for continued GP-MSK-MRI access.…”

“…For example, the National Health Service England is introducing physiotherapists as musculoskeletal first-contact practitioners; however, this is dependent on the recruitment of a significant number of clinicians with the associated challenges [7]. Mobile health (mHealth) technology is proposed as a cost-effective solution for improving health care delivery [12,13]. Although many mHealth tools have not demonstrated cost-effectiveness or have shown merely to shift spending to another part of the health system [14], it would seem logical that a digital alternative to physio-led triage, able to replicate the same stratification of care and reduction in costs, is a desirable objective.…”

“…Those in these groups may also wish to be involved in the conduct and dissemination of findings from a related review (as PPI contributors). Other reviews we have conducted have involved patients/the public [ 18 , 19 ]; such involvement was successful in helping us to think about data emerging from a review and in providing us with feedback on the presentation and interpretation of its findings.…”
