A Survival Analysis with Identification of Prognostic Factors in a Series of 110 Patients with Newly Diagnosed Glioblastoma Before and After Introduction of the Stupp Regimen: A Single-Center Observational Study

Abstract: Adoption of the Stupp protocol had a favorable impact on overall, but not on progression-free, survival rate. Wider surgical resection involving the peritumoral brain zone, as confirmed by univariate and multivariate analysis, represents the most favorable prognostic factor.

“…Univariate analyses showed statistically significant improvement in overall survival of patients younger than 50 years (median survival 19 months) in comparison with the group of patients older than 50 years (median survival 7 months) (p < 0.00045) ( Table 3). This result is in agreement with previous findings that overall survival is significantly higher for younger groups of patients (<50) in comparison with older groups (>50) [19,[24][25][26]. Disruption of the DNA methylation mechanism is associated with aging and is characterized by genome-wide hypomethylation and promoter specific methylation [26][27][28].…”

“…The MGMT DNA repair enzyme counteracts the cytotoxic effect of DNA alkylating agents such as TMZ and mediates chemoresistance. The introduction of TMZ treatment and Stupp regimen in Serbia significantly improved the survival rates of glioblastoma patients [19][20][21]. The survival outcome of the diffuse glioma patients from Serbia included in this study was not correlated with MGMT promoter methylation, which corresponds with findings from a recent study dealing with GBM patients in Serbia [19].…”

“…Firstly, the data came from several databases with limited sample size. Secondly, the prognostic factors in the nomogram did not contain the extent of GBM surgical resection which is a cardinal factor associated with GBM prognosis . Therefore, more samples with detailed clinical information will be collected to make up for these shortcomings and assess the efficiency of the nomogram in future study.…”
