Purpose: In persons with a hip or knee flexion contracture 25 , fitting a prosthesis is said to be difficult. This systematic review aims to assess the evidence for fitting of a prosthesis in persons with a severe contracture (25) after a lower limb amputation. Method: PubMed, Embase, Scopus, CINAHL, and Orthotics & Prosthetics Virtual Library databases were searched from inception to December 2019, using database specific search terms related to amputation, prosthesis, and contracture. Reference lists of included studies were checked for relevant studies. Quality of the included studies was assessed using the critical appraisal checklist for case reports (Joanna Briggs Institute). Results: In total, 13 case studies provided evidence for fitting of a prosthesis in more than 63 persons with a transtibial amputation and three with a transfemoral amputation, all of whom had a hip or knee flexion contracture 25. Some studies found a reduction in contractures after prosthesis use. Conclusions: Several techniques for fitting a prosthesis in case of a flexion contracture 25 were found. Contracture reduction occurred in some cases and was possibly related to prosthesis use. Fitting a transtibial or transfemoral prosthesis in persons with a lower limb amputation with a severe flexion contracture is possible. ä IMPLICATIONS FOR REHABILITATION This study provides information on prosthesis prescriptions and adaptations for persons with a transfemoral and transtibial amputation with a flexion contracture 25. The fitting of bent prostheses is not limited by prosthetic components and techniques. Parallel to the use of bent prostheses, it is also important to treat the contracture.

This study describes 49 young amputees who either have attended or have been operated at the Orthopaedic Department of Dodoma Regional Hospital from 1983 to 1991. Emphasis has been put on the aetiological factors of 56 amputations under review, underlying those that are typical of the Least Developed Countries in Africa and highlighting how they interrelate with the beliefs and traditions of the African society. Apart from 14 amputations performed for trauma of different type, important roles were played by home environment, tumours, wild animal bites and the health system itself. Nine amputations have been carried out for burns, while complications after medical care, particularly operative, have called for 12 amputations. Bone tumours have been responsible for eight and hyena assaults and snake bites have resulted in seven amputations. Special interest has been raised by four amputations performed for the so-called 'idiopathic tropical lower limbs gangrene' whose aetiology is still unknown.

The records of Dodoma Orthopaedic Department, Tanzania were reviewed for the period July 1986 to December 1990 in order to identify the reasons for revision surgery. A total of 26 patients required revision surgery. Two main groups were identified. In the first group 4 patients had a higher level of amputation because of gangrene. In the second group 22 patients had revision surgery because of other stump defects caused by technical mistakes when carrying out the original amputation, or other complications.

Abstract: A survey of 143 amputations performed at Dodoma Regional Hospital, Tanzania between 1983 and 1988 is presented. Consideration is given to indications for amputation, amputation levels, stump revision and supply of prostheses.
