A super-agonist of growth hormone–releasing hormone causes rapid improvement of nutritional status in patients with chronic kidney disease

Abstract: Chronic kidney disease is frequently associated with protein-energy wasting related to chronic inflammation and a resistance to anabolic hormones such as insulin and growth hormone (GH). In this study, we determined whether a new GH-releasing hormone super-agonist (AKL-0707) improved the anabolism and nutritional status of nondialyzed patients with stage 4-5 chronic kidney disease randomized to twice daily injections of the super-agonist or placebo. After 28 days, this treatment significantly increased 24-h GH… Show more

“…The use of recombinant human GH (rhGH) and rhIGF-1 in CKD patients is an obvious choice with some potential to improve muscle protein turnover and nitrogen retention. In children with CKD and growth retardation use of GH and IGF-1 has demonstrated to have positive effects on growth and outcome [70] and recently a newer GH releasing hormone ‘super-agonist’ has been utilised in stage 4/5 CKD patients showing significant positive effects on anabolism and LBM [77]. …”

“…Whilst pharmacologically correct, such an approach can be cumbersome and draw focus away from the endogenous agonist. This has led to some commentators to coin the term 'super agonism' to describe situations A c c e p t e d M a n u s c r i p t 14 where synthetic orthosteric ligands that share an overlapping binding site with the endogenous ligand(s) have been demonstrated to activate the receptor with higher efficacy than the endogenous agonist (Niemczyk et al, 2010, Tan et al, 2002.…”

“…In renal failure phosphorylation of JAK2 and the downstream signaling molecules STAT5, STAT3 and STAT1 are impaired as is the nuclear levels of phosphorylated STAT proteins. It showed very promising results with rapid improvement of nutritional status in these patients [86]. studies would seem to be of major importance.…”
