A Study On The Clinical Profile And Management Of Inverted Papilloma

BackgroundThe presence of a mass in the nose and paranasal sinuses may seem to be a simple problem; however it raises many questions about the differential diagnosis. The aim of this study is to evaluate the clinico-pathological profile of sinonasal masses in our environmentThis is a retrospective analytical review of all the patients with sinonasal masses that presented to the national ear care center, Kaduna over a six year (2003-2008) period. Their biodata, clinical profile and histological diagnoses were analyzed.FindingsA total of 76 patients were analyzed, age range 5 to 64 yrs with a mean age of 33.3 yr median and modal age of 35.00 (SD = 13.1 ± 1.5). Majority of the patients were in the age groups 21-50 yrs. There were 34 male and 42 female with M: F ratio of 1:1.2. The main presenting symptoms are nasal blockage 97.4% and rhinorrhea 94.7%. It was bilateral in 34 (44.7%), left side in 24(31.6%) and right side in 18(23.7%) patients. The commonest clinical diagnoses were simple nasal polyp 47(61.8%) and antrochoanal polyp 10(13.2%). About 59 (77.6%) were benign, 2 (2.6%) were malignant and 15 (19.7%) were lost to follow up. The commonest histological diagnosis is simple inflammatory nasal polyp in 28 (36.8%) patients and the least was nasal capillary hemangioma 2 (2.6%). About 55(72.4%) patients had surgical treatment.ConclusionsNasal obstruction and rhinorrhea are the commonest symptoms of presentation, simple inflammatory nasal polyp is still the commonest histological pattern seen in our environment, and surgery is still the best modality of treatment for benign tumor thus the need for advocacy for early recognition and referral to the ENT surgeon.

Background: Pathological lesions of the paranasal sinuses include a wide spectrum of conditions ranging from inflammation to neoplasms both benign and malignant. Most patient of common cold present with symptoms of nasal discharge, nasal obstruction, headache and nasal allergy etc. The patient of paranasal mass usually present with facial deformity, swelling or repeated episodes of epistaxis. The aim of the study was to establish the role of CT in evaluation of pathologies and their proper early diagnosis.Methods: It is a prospective study. A total of 100 patients who were referred to our department with clinical suspicion of PNS disease underwent CT evaluation of PNS using 64 multi slice CT scanner from December 2012 to October 2015. Results: Out of 100 cases 51 (51%) cases were males and rest 49 (49%) patients were females. The majority of the cases were of age group 16-30 which were 37case (37%) The most common symptoms were nasal obstruction (50%), followed by nasal discharge (49%), headache in 20% cases. Most common anatomical variations seen was deviated nasal septum (49%) more commonly on right side next common was agger nasi in 48% of cases. Maxillary sinuses are most commonly involved in the study (82 cases), followed by the ethmoid sinuses (ant. group 77 cases, post group 40 case) and frontal sinus 55 cases. The most common pathology seen was masses in 30% cases followed by DNS in 21% cases. The most common form of mucosal thickening noted is circumferential type seen in 7% cases. The most common pathology involving the sinuses was sinusitis (30%) followed by polyp (25%).Conclusions: Most of the patients with PNS pathology were from 2nd and 3rd decade. Slight male preponderance was noted. Majority of the patients presented with nasal obstruction. Most common anatomical variant seen was DNS. Most common sinus involved was maxillary. Sinonasal pathologies were the most common followed by pure nasal or pure sinosal. The most common pathology was inflammatory (74%). The most common benign pathologies were polyps with 92% accuracy. 15% of cases had malignancy with diagnostic accuracy of 90%.

Introduction: The inverted papilloma (IP) is a rare and benign sinonasal tumour, bearing an incidence of 0.75-1.5 cases per 100 thousand inhabitants/ year(representing 0.5-4%) of nasal tumours and 91-99% of cases are unilateral .IP is locally aggressive tumour having propensity for recurrence and association with malignancy , We Report a rare case of inverted papilloma in bilateral nasal cavity. Case Report: A 54 Years old man presenting with history of bilateral nasal obstruction. CT scan Nose and PNS revealed soft tissue density mass in bilateral nasal cavity, Intra-operative bilateral ethmoid sinuses , Lt. maxillary , Lt. sphenoid sinus were found involved, Total removal was done endoscopically , On the basis of clinic-pathological findings , The tumour was diagnosed as inverted papilloma , On follow up patient was disease free. Conclusion: The very rarity with which inverted papillomas affect the nasal cavities (bilateral) makes this report so important, especially considering post-operative follow up
