The aims of our study were to describe our case series of three urethral diverticulum carcinomas (UDC) in women and to review the literature on UDC in females to determine patient characteristics, presenting symptoms and outcomes along with optimal investigations and treatment modalities. A literature search was performed utilizing Medline, EMBASE and the Cochrane library for all papers including case reports on UDC in women published to date. The results along with those of our three cases are detailed. A total of 126 cases of UDC in women have been reported; 75% adenocarcinoma (Adenoca), 15% transitional cell carcinoma (TCC) and 10% squamous cell carcinoma (SCC). Median age at presentation was 53 years (range, 14–81 years). The commonest presenting symptoms were bleeding and retention. Cystoscopy, MRI and trans-urethral biopsy were the commonest methods of diagnosis. Treatment was radiotherapy +/− chemotherapy alone in 21%, local excision +/− radiotherapy in 44%, urethrectomy in 3% and anterior exenteration +/− radiotherapy in 32%. At last follow-up 63% were alive and well, 10% were alive with recurrent cancer and 25% had died from their disease. UDC is rare in women. It is predominantly adenocarcinoma. There is no established treatment and survival is at best moderate. An international registry and consensus on management is needed if this is to be improved.

Radiation therapy in carcinoma of the female urethra

In the interval from 1934 to 1966, 41 patients with the diagnosis of carcinoma of the female urethra were seen in the Ontario Cancer Institute. The data on 31 patients are sufficiently complete to report, with 25 patients being treated definitively and 6 palliatively. Carcinomas of the meatus are best treated by interstitial gamma ray therapy in dosages between 5500 to 6000 rads. Surgical excisions or figuration of meatal lesions should be followed by definitive local interstitial radiotherapy when there is a question of residual disease. Involvement of the distal half of die urethra without extraurethral spread can be handled in a similar fashion. Tumors of the proximal urethra or entire urethra carry a serious prognosis and should be treated by both external radiation as well as interstitial radium techniques. Treatment (surgical or radiation) to the regional lymph nodes should be reserved for demonstrated disease either on clinical examination or by lymphangiography.

A study of urethral tumors in men and women has been made. The 3 most important tumors involving the male urethra are: 1. those arising in the meatus and parameatal area; 2. squamous cell cancer of the membranous urethra; and 3. transitional cell cancer of the prostatic urethra. The latter 2 respond poorly to any and all forms of therapy regardless of extent of ablative surgical attempt or radiation therapy dosage. Tire' latter prostatic urethral tumor is also hormone‐resistant. The most common tumors of the female urethra are epidermoid carcinoma and adenocarcinoma. Tumors of the distal urethra may be successfully treated by partial urethrectomy or by irradiation. For proximal or entire urethral tumors, the prognosis with radiation or surgery alone is poor; combined radiation and surgery is preferable.
