ÖzetAmaç:Manyetik rezonans görüntüleme (MRG) tibbin tüm alanlarinda hizla gelişen önemli bir tani araci olmuştur. Fakat son çalişmalarda MRG uygulanan hastalarin %37'sinde şiddetli anksiyete ve korku oluşumu raporlanmiştir. Çalişmamizda hastalara video bilgilendirme yaparak anksiyete düzeylerini azaltmayi amaçladik. Gereç ve yöntem: Aralik 2015 ve Şubat 2016 tarihleri arasinda 1.5 Tesla MRG cihazi kullanarak yaptiğimiz çalişmamiza klinisyen tarafindan MRG için yönlendirilen 100 hasta dahil edildi. Hastalar video bilgilendirme grubu (müdahale grubu, n=50) ve kontrol grubu (n=50) olmak üzere ikiye ayrildi. Tüm hastalarin anksiyete düzeyleri, Durumluk-Sürekli Kaygi Envanteri (STAI) ve Görsel analog skalasi (GAS) uygulanarak belirlendi. Daha önce hazirlanmiş olan 3 dakikalik video, müdahale grubundaki hastalara MRG öncesinde izletildi. Video sonrasi bu gruba STAI-S ve GAS tekrar uygulandi. MRG sonrasi ise tüm hastalara belirlediğimiz 7 sorudan oluşan anket uygulandi ve verdikleri cevaplar kaydedildi. Bulgular: Yaptiğimiz araştirma sonunda video bilgilendirme yapilan hastalarla video bilgilendirme yapilmayan hastalar arasinda MRG öncesi anksiyete değerleri arasinda anlamli bir fark bulunmadi. Video bilgilendirme sonrasinda ise bu hastalarin GAS ve STAI-S skorlarinda anlamli bir azalma görüldü (p=0.002). MRG sonrasi yapilan ankette; müdahale grubundaki hastalarin kontrol grubuna göre MR süresi ile ilgili endişelerinin anlamli bir şekilde daha az olduğu (p=0.016) ve beklediklerinden daha iyi geçtiği görüldü (p=0.022). Sonuç: Maliyet kaybina yol açmayan ve zararsiz olan bu uygulamanin hasta anksiyetesini düşürmede başarili olduğu gösterildi. AbstractPurpose: Magnetic resonance imaging (MRI) has been the most important and rapidly developing diagnostic tool in all spheres of medicine. However studies recently reported that severe anxiety and fear occurs in 37% of patients who underwent MRI. In our study, we aimed to reduce patient anxiety by video information. Materials and methods: Between December 2015 and February 2016 using a 1.5 Tesla MRI machine, 100 patients were enrolled in the study. Patients were divided into two groups; video information group (n = 50) and control group (n = 50). Anxiety levels of the patients were determined by applying State-Trait Anxiety Inventory (STAI) and Visual Analogue Scale (VAS). A three-minute video, which was prepared earlier, was watched by the patients in video information group before MRI. After video information, STAI-S and VAS were applied again. Subsequent to MRI, all patients were given a questionnaire consisting of seven questions and their responses were recorded. Purpose:To prospectively assess and compare two formulations and methods of administration of low-dose nasal midazolam for the treatment of claustrophobic patients undergoing magnetic resonance imaging (MRI) as part of a multicenter Phase III trial. Materials and Methods:In all, 108 consecutive adult claustrophobic patients were randomly assigned to one of two treatment groups (multidose group: MDG, unit-dose group: UDG). MDG encompassed 55 patients who received intranasally a 0.5% midazolam formulation into each nostril (total dose, 1.0 mg), whereas the 53 patients in UDG received a 1% midazolam formulation into only one nostril (total dose, 1.0 mg). This initial dose could be repeated once. Patient tolerance and anxiety were assessed using a questionnaire and a visual analog scale immediately before and after MRI. Image quality was evaluated using a fivepoint scale.Results: In all, 53/55 MR examinations (96%) with MDG and 52/53 (98%) with UDG were completed successfully. The dose of 1 mg had to be repeated significantly less often in UDG compared to MDG (4/53, 8% vs. 13/55, 24%; P ϭ 0.003). The image quality of all MR examinations was rated good to excellent, and slightly better in UDG (P ϭ 0.045).Conclusion: Nasally applied low-dose midazolam is a patient-friendly solution to facilitate MRI of claustrophobicpatients. The nasal spray of UDG is superior to that of MDG with a necessity of additional dosing.

SUMMARY:Fetal and neonatal MR imaging is increasingly used as a complementary diagnostic tool to sonography. MR imaging is an ideal technique for imaging fetuses and neonates because of the absence of ionizing radiation, the superior contrast of soft tissues compared with sonography, the availability of different contrast options, and the increased FOV. Motion in the normally mobile fetus and the unsettled, sleeping, or sedated neonate during a long acquisition will decrease image quality in the form of motion artifacts, hamper image interpretation, and often necessitate a repeat MR imaging to establish a diagnosis. This article reviews current techniques of motion compensation in fetal and neonatal MR imaging, including the following: 1) motion-prevention strategies (such as adequate patient preparation, patient coaching, and sedation, when required), 2) motion-artifacts minimization methods (such as fast imaging protocols, data undersampling, and motion-resistant sequences), and 3) motion-detection/correction schemes (such as navigators and self-navigated sequences, external motion-tracking devices, and postprocessing approaches) and their application in fetal and neonatal brain MR imaging. Additionally some background on the repertoire of motion of the fetal and neonatal patient and the resulting artifacts will be presented, as well as insights into future developments and emerging techniques of motion compensation.ABBREVIATIONS: bFFE ϭ balanced fast-field echo; FLASH ϭ fast low-angle shot; G RO ϭ readout gradient; NSA ϭ number of signal averages; PROPELLER ϭ periodically rotated overlapping parallel lines with enhanced reconstruction; RF ϭ radio-frequency M R imaging is an ideal diagnostic technique for the evaluation of infants and fetuses 1-7 because of the absence of ionizing radiation, the superior contrast of soft tissues compared with sonography, and the availability of different contrast options (T1-weighted, T2-weighted, and diffusion-weighted imaging, Fig 1) to improve characterization of both anatomy and pathology. However MR imaging remains a relatively slow technique, with scanning times for most applications in the order of seconds to minutes, leaving them susceptible to motion artifacts. The normally mobile fetus and the unsettled neonate present a major difficulty because the presence of motion during a long acquisition will decrease image quality in the form of motion artifacts (Fig 2), hamper accurate image interpretation, and often necessitate a repeat MR imaging to establish a diagnosis. This may have major emotional implications for parents and can stress the tight budgets of health care providers.
