A Study of Pathologic Risk Factors in Postchemoreduced, Enucleated Specimens of Advanced Retinoblastomas in a Developing Country

Abstract: Context.—Advanced cases of retinoblastoma are treated with chemoreduction followed by enucleation. Further adjuvant therapy is recommended in patients with known pathologic risk factors (PRFs). Objectives.—To determine the PRFs in enucleated specimens after chemoreduction and their association for adverse events of recurrence, metastasis, or death. Design.—This was a retrospective study of 77 enucleation specimens from patients treated between January 2000 and September 2008 with … Show more

“…The chemoreduction protocol could have been responsible for pathological downstaging of tumor as suggested by Zhao et al [32]. Overall, HRF in postchemotherapy-reduced eyes were found in 10.1% (27/228) in our study, which is much lower than 51.9% reported by Suryawanshi et al [33]. This gross difference is due to the inclusion of vitreous seeds in 44.2% as HRF and consideration of even extraocular cases for the study of HRF.…”

“…Previous reports have described HRH after enucleation, 13,18 but ours is the largest cohort of primary and secondary enucleation from a single institution. In this study we reviewed 207 enucleated eyes of 202 patients with retinoblastoma; 59 eyes (29%) were considered to have HRH: 46 primarily enucleated eyes and 13 secondarily enucleated eyes.…”

“…Our incidence of HRH is comparable with previous reports. 7,11,14 One report of a higher incidence of HRH 18 was due to more advanced (pT4) disease at diagnosis.…”

“…Aqueous seeding (see 6.2.4.1) with or without concomitant anterior uveal involvement was until recently considered an intractable form of retinoblastoma. The prognostic value of aqueous seeding and/or anterior uveal invasion such as iris or corpus ciliaris is controversial, but is no longer considered to be high risk for metastasis and, consequently, does not represent an automatic indication for adjuvant chemotherapy (Baroni et al, 2014;Sreelakshmi et al, 2017;Suryawanshi et al, 2011).…”
