Prevalence of Polypharmacy, Hyperpolypharmacy and Potentially Inappropriate Medication Use in Older Adults in India: A Systematic Review and Meta-Analysis

Background: Older people often receive multiple medications for chronic conditions, which often result in polypharmacy (concomitant use of 5‒9 medicines) and hyperpolypharmacy (concomitant use of ≥10 medicines). A limited number of studies have been performed to evaluate the prevalence of polypharmacy, hyperpolypharmacy, and potentially inappropriate medication (PIM) use in older people of developing countries. The present study aimed to investigate regional variations in the prevalence of polypharmacy, hyperpolypharmacy, and PIM use in older people (60 + years) in India.Methods: Studies were identified using Medline/PubMed, Scopus, and Google Scholar databases published from inception (2002) to September 31, 2020. Out of the total 1890 articles, 27 were included in the study.Results: Overall, the pooled prevalence of polypharmacy was 49% (95% confidence interval: 42–56; p < 0.01), hyperpolypharmacy was 31% (21–40; p < 0.01), and PIM use was 28% (24–32; p < 0.01) among older Indian adults. Polypharmacy was more prevalent in North-east India (65%, 50–79), whereas hyperpolypharmacy was prevalent in south India (33%, 17–48). Region-wize estimates for the pooled prevalence of PIM use in India were as follows: 23% (21–25) in East, 33% in West (24–42), 17.8% in North (11–23), and 32% (26–38) in South India. The prevalence of PIM use in adults aged ≥70°years was 35% (28–42), in those taking more medications (≥5.5/day) was 27% (22–31), and in adults using a high number of PIMs (≥3) was 29% (22–36). Subgroup analysis showed that cross-sectional studies had a higher pooled prevalence of polypharmacy 55% (44–65) than cohorts 45% (37–54). Hyperpolypharmacy in inpatient care settings was 37% (26–47), whereas PIM use was higher in private hospitals 31% (24–38) than government hospitals 25% (19–31).Conclusion: Polypharmacy and hyperpolypharmacy are widely prevalent in India. About 28% of older Indian adults are affected by PIM use. Objectives: The objective of our study is to evaluate the drug utilization pattern among geriatric patients in a tertiary care teaching hospital in South India and analyze the prescription pattern. Methods: Retrospective medical record analysis was performed for indoor cases of the geriatric patients (aged ≥65 years) admitted in medicine wards of Raichur Institute of Medical Sciences teaching hospital, Raichur, from June 2015 to May 2016. They were analyzed for demo graphics, disease pattern by system involved, disease diagnosed, comorbidity pattern, most commonly prescribed drugs, distribution of drugs according to their therapeutic class, evaluation of polypharmacy, route of administration, fixed-dose combinations, and the World Health Organization core prescribing indicators. Results: A total of 400 prescriptions of geriatrics patients admitted in inpatient medicine department were randomly selected and all the required data for the study were collected and evaluated. The majority of patients were found to be in the age group of 65–70 years (70.25%). It was noticed that females (51.75%) were more compared to males (48.25%). The drugs prescribed were analyzed. Ranitidine, ceftriaxone, theophylline, and salbutamol were prescribed to majority of the patients in this study group. The average number of drugs per prescription was 7.42; drugs prescribed by generic name were 33.62%. Among the drugs prescribed, 88.69% were from the National List of Essential Medicines. Total encounters having inject table preparations were 100% and total encounters having antibiotic prescribed were 91%. The prevalence of cardiovascular disorders (32.5%) was high followed by respiratory disorders (32.25%). Conclusion: This study provides insight into the patterns of drug use in our tertiary care hospital on geriatric patients as well as various diseases prevalent among geriatric patients. Widespread polypharmacy, high use of antibiotics and injections, and lack of generic prescribing are some concerns that need to be addressed to promote rational usage of drugs.

Background: Geriatrics are the rapidly growing age group suffering with two or more chronic diseases leading to polypharmacy. Polypharmacy can boost the risk of adverse events, drug interactions and medication no adherence. This study aims at investigating the prevalence, different factors and consequences associated with polypharmacy in geriatrics. Methodology: A prospective observational study was carried out for a duration of nine month in a private corporate hospital, Coimbatore, Tamil Nadu. Patients above 65 years were included in the study. The collected data was investigated for polypharmacy and analyzed for factors and consequences associated with polypharmacy. Results: A total of 344 patients were included in the study. The average age of the study population was found to be 72.9 ± 6.5. Average hospital stay of the study population was found to be 10.22±8.7 days. A total of 2034 drugs were prescribed for the study population. The average number of drugs prescribed for the study population was found to be 9.68 (±1.94) with minimum of 5 drugs to the maximum of 14 drugs during the study period, 110 (52%) among the study population were found to be prescribed with potentially inappropriate medications. The mean value of drug related problems in the study population was found to be 1.81±1.3. Conclusion:The prevalence of polypharmacy and potentially inappropriate medications use were very high among the study population. This study assured the need of close monitoring of patients under polypharmacy since it can lead to higher number of drug related problems and increased hospital stay.
