A structured, extended training program to facilitate adoption of new techniques for practicing surgeons

Abstract: Our comprehensive educational program led to full and sustained adoption of TEP for those who completed the course. Time constraints, travel costs, and case volume are major considerations for successful completion; however, the program is feasible, acceptable, and affordable.

“…29 Targeted training interventions involving simulation or expert coaching, especially for busy surgeons already in practice, may increase adoption and have shown success in laparoscopic ventral hernia repair, as well as in inguinal hernia on a smaller scale. 30,31 In contrast, recommending training and education for all surgeons not currently performing MIS is not feasible and is unlikely to result in the desired practice change. For patients with bilateral or recurrent inguinal hernias, healthcare delivery system reform may be the preferred intervention to improve access and increase guideline-concordant practice.…”

“…Full length videos were used in the Wisconsin Surgical Coaching Program. 9 However, despite being a small group of dedicated volunteer coaches (n ¼ 8) and a small group of eager individuals desiring to be coached (n ¼ 12), none of the groups completed the program as planned with 4 full video reviews over the year. Participants cited workload and time constraints as one of the primary reasons for not completing the program as planned.…”

“…6e8 In addition, surgical videos have also been used to facilitate surgical coaching. 9 It is notable that when surgical video is used in these capacities, there is significant variability in the implementation procedures. Moreover, despite the growing use of surgical videos, there is still a notable gap in the use of video for formative and summative assessment.…”

“…In healthcare, data to support coaching have been generated primarily in the context of clinical quality and patient safety. 45,51,52 Coaching has been shown to improve clinicians' adherence to evidence-based practice guidelines, 53,54 to support expansion of surgeons' breadth of practice, 55 and to improve surgeons' technical skills in simulation settings. 17 Evidence is emerging to support coaching in surgery.…”
