A simplified method for stent placement in the distal duodenum: Enteroscopy overtube

Abstract: The treatment of choice for patients with unresectable neoplastic obstruction of the small intestine is the placement of expandable metal stents. However, endoscopic delivery from the distal duodenum can be more difficult. This case, shows the usefulness and technical advantages of the overtube and single balloon enteroscopy in the treatment of neoplastic stenosis affecting the small intestine.

“…In our case, we used an overtube as an extralarge working channel. Although a similar technique using spiral single-balloon enteroscopy and spiral enteroscopy was previously described,7-9 our case is of additional interest because we describe in detail all the steps necessary to perform a balloon overtube placement of a SEMS into the proximal small bowel. In addition, limited data are available on this technique 10…”

“…Data regarding this technique are currently limited to case reports and small series (Table 1). [13][14][15][16][17][18][19][20] The technique involves the use of the dedicated balloon-assisted or spiral overtubes as conduits for placement of esophageal or enteral SEMS through malignant mid-gut obstruction under fluoroscopic guidance (Fig. 2).…”

“…However, SEMS placement for malignant small bowel obstructions is little-known and also more challenging. A deep small bowel enteroscopy is limited, and three endoscopy systems are now available: [double balloon endoscopy (DBE), single balloon endoscopy, and spiral endoscopy[5-7]]; however, such endoscope systems do not have working channels large enough for the stent delivery systems to pass through. Therefore, the standard through-the-scope (TTS) technique for stent deployment could not be applied.…”
