A Short‐Term Long‐Chain Triglycerides Infusion Has No Influence on Immune Function of Adult Patients Undergoing Gastrointestinal Surgery

Abstract: The regimens of LCT administration may have diverse effects on human immune function in different patient populations. However, LCT emulsion at an appropriate dose and infusion speed does not alter human immune function of adult patients undergoing moderate gastrointestinal surgery.

“…Table 4 reports on immuno-inflammatory and clinical outcomes of trials comparing the use of SOLE in PN with lipid free PN admixtures. Some studies reported either no change or immunostimulatory effects when SOLE was used compared to lipid-free PN admixtures in surgical patients (Dionigi et al, 1985;Monson et al, 1986;Li et al, 2007;Kagawa et al, 2013). Li et al (2007) and Kagawa et al (2012) suggested LE infusion is dosedependent and the infusion rate at 0.09-0.12 g/kg/h does not alter human immune function (Li et al, 2007;Kagawa et al, 2013).…”

“…Some differences between these studies are the observed changes in insulin and glucose concentrations induced by infusion of intralipid. For example, when systemic insulin increases, an enhancement of systemic cytokines occurs [1113], whereas if insulin concentrations are kept stable with a hyperinsulinaemic–euglycaemic clamp, no effect on the systemic cytokine response occurs [14, 15]. Thus, as demonstrated in our study, insulin level control is critically important [46].…”

“…The effect of NEFAs on inflammation has been investigated especially in models where inflammation is induced experimentally by infusion of endotoxin, with additional NEFA infusion enhancing the systemic cytokine response in some [1113], but not all studies [14, 15]. Some differences between these studies are the observed changes in insulin and glucose concentrations induced by infusion of intralipid.…”

“…Studies concerning the effect of LCTs on neutrophil function, including phagocytosis, are numerous but results are equivocal and consist of stimulatory, inhibitory as well as neutral effects [7,29–33]. Studies on the use of LCT‐MUFA and VLCT are more sparse.…”
