SUMMARY:This work aims to investigate the anatomical basis and clinical application value of renal pedicle locating in retroperitoneoscopic nephrectomy. To summarize the anatomical basis of renal pedicle locating through retrospective analysis of 278 cases of retroperitoneoscopic nephrectomy from July 2007 to September 2009, during which renal pedicle was located at about 2-4 cm below the medial arcuate ligament of the diaphragm in the space between the psoas major muscle and inferior vena cava (abdominal aorta) in the anatomical level of space before psoas. The operation of 278 patients was all successfully completed, where renal pedicle was quickly found. It took 3.5±1.3 min to locate the renal pedicle, and 95.6±23.8 min to operate. In retroperitoneoscopic nephrectomy, it is most preferable to locate renal pedicle in the space before psoas. The renal pedicle is located exactly at about 2-4 cm below the medial arcuate ligament of the diaphragm in the space between the psoas major muscle and inferior vena cava (abdominal aorta). The time for locating the renal pedicle can be shortened if the surgeon is familiar with the anatomic features of renal pedicle in retroperitoneoscopy, thereby saving the operation time.

Laparoscopic surgery has become a preferred treatment option for the management of upper urinary tract pathology. Large series of complex urologic operations have been published, with favorable outcomes. Although the early experience with urologic laparoscopy consisted almost exclusively of transperitoneal procedures, some surgeons prefer the retroperitoneal approach. The debate of retroperitoneal versus transperitoneal laparoscopy for upper tract pathology is complex because comparative and randomized studies are lacking. In contrast to the retroperitoneal approach, transperitoneal laparoscopic procedures allow for a larger working space, adequate maneuverability, and the presence of familiar anatomic landmarks. This article highlights our experiences and approaches to major laparoscopic upper urinary tract surgery for benign and malignant conditions. A summary of the published literature as it pertains to the transperitoneal approach also is presented.

RESUMENNEFRECTOMÍA RADICAL LAPAROSCÓPICA Revisamos en este artículo los distintas abordajes quirúrgicos para realizar una nefrectomía radical laparoscópica, la vía transperitoneal, la retroperitoneal, así como la mano-asistida.Describimos las ventajas e inconvenientes de cada una de estas alternativas así como resumimos los citas bibliográficas más importantes en la literatura médica con respecto a las mismas.A pesar de que es una cirugía con una corta existencia, menos de 15 años, ha conseguido convertirse en un estándar de tratamiento y hoy puede considerarse en muchos centros como la cirugía de elección en tumores de riñón T1 y T2.Palabras clave: Laparoscopia. Nefrectomía radical. ABSTRACT RADICAL LAPAROSCOPIC NEPHRECTOMYIn this article, we review the different surgical approaches to carry out radical laparoscopic nephrectomy: transperitoneal approach, retroperitoneal approach and hand-assisted approach. We describe the advantages and drawbacks of each alternative and summarize the most important references in the medical literature. In spite of this being a relatively new surgical approach, less than 15 years old, it has become a standard treatment and, today, is considered as the elective surgical treatment for T1 and T2 renal tumours in many centres.
