A severe case of olanzapine overdose with analytical data

Abstract: This study reports an acute olanzapine monointoxication with severe toxicity and high whole blood olanzapine concentrations. Clinical and analytical data of similar samples obtained in non-fatal life-threatening cases can be very useful when interpreting postmortem cases.

“…Olanzapine resembles clozapine both chemically and pharmacologically, although monitoring for agranulocytosis is not required with olanzapine because this effect is much less common (29,30). Overdoses have been characterized by CNS depression and unpredictable fluctuation between sedation and agitation (29,31).…”

“…5,6 Therapy with olanzapine has been linked to the development of myopathy, occasionally associated with massive rises in serum CK. 7 Á 12 Rhabdomyolysis has also been reported in the context of acute olanzapine overdose, 13,14 as have neuroleptic malignant syndrome, 15 Á 18 serotonin syndrome, 19,20 miosis, 5,21 and extrapyramidal disorders. 22 Clinical features of toxicity may persist for up to 24 hours after acute olanzapine ingestion.…”

“…Similar findings were reported by Arora et al ( 7 ), who presented a case of a patient who ingested 1600 mg and required minimal therapy. There are reports of patients surviving and completely recovering from an overdose with 800 mg of olanzapine and serum concentration of 0.991 mg/L or overdosing with 550 mg and 280 mg which led to serum concentrations of 0.815 mg/L and 0.41 mg/l, respectively ( 3 , 18 , 19 , 20 ).…”
