A ruptured dissecting aneurysm of the vertebral artery: comparison of angiographic and histological findings

Abstract: We present autopsy findings in a patient with a dissecting aneurysm of the vertebral artery causing subarachnoid haemorrhage. We sectioned the artery longitudinally and compared it with the angiogram, which showed the "pearl-and-string". Histological examination showed a pseudoaneurysm covered by adventitia alone, forming the "pearl". Internal elastic lamina and media were destroyed, and haematoma extended in the subadventitial space of the wall of the pseudoaneurysm. Media thickened by haematoma caused the "s… Show more

“…In our case 2, we did not consider embolization because many PICA aneurysms are dissecting- or pseudo-aneurysms,[111523] and thus are prone to perforation during coil embolization. [81018] However, our findings in case 1 suggest that the aneurysm in case 2 was a true aneurysm; therefore, endovascular treatment was an appropriate treatment option. We performed selective aneurysmal coil embolization and completed the procedure successfully without perforation.…”

“…In these diseases, myxoid degeneration of the wall may worsen the arterial damage, most commonly in the form of dissections. Some authors have found the presence of myxoid degeneration in apparently healthy arteries [10, 11, 12]. Our patient had no signs or history of the above-mentioned clinical entities.…”

“…With regard to extracranial arteries, there have been two reports of nondissecting aneurysms caused by myxoid degeneration in the cervical portion of the carotid artery [8, 9]. Four cases of intracranial dissecting aneurysms in adults have also been reported [10, 11, 12, 13]. Therefore, intracranial aneurysms with myxoid degeneration are extremely rare.…”

“…With the formation and accumulation of mural thrombus, the injured vertebral artery was occluded in the end. [1][2][3][4] The segment of injury occurred most frequently on the level of fracture and dislocation, the foramen transversarium of C 6 where the vertebral artery entered.…”
