Locally advanced non-small-cell lung cancer still represents a “grey zone” in terms of the best treatment choice and optimal clinical outcomes. Indeed, most patients may be suitable to receive different treatments with similar outcomes such as chemo-radiotherapy (CHT-RT) followed by immunotherapy (IO) or surgery followed by adjuvant local/systemic therapies. We report a clinical case of a patient submitted to primary thoracic surgery who developed a mediastinal nodal recurrence successfully treated by CHT-RT-IO. Subsequently, a single brain lesion was found to have been successfully treated by single fraction stereotactic ablative radiotherapy. The patient is still on follow-up and she is free from disease having a good quality of life. In this report, we also perform a mini review about the role of CHT-RT followed by IO in treating loco-regional relapse after surgery. The role of SABR after IO is also evaluated, finding that it is safe and well tolerated. More robust and larger clinical data are needed in this particular setting to better define the role of the combination of systemic and local treatments in the management of intrathoracic and intracranial relapse for patients already submitted to CHT-RT followed by immunotherapy.

Crinum pedunculatum (R.Br) has been used by traditional medicine practitioners in Ghana for the topical management of inflammation and associated disorders. This study investigated the anti-arthritic activity of the methanol, ethanol and ethyl acetate extract of the bulbs of Crinum pedunculatum using Sprague Dawley rats. The anti-arthritic activity of Crinum pedunculatum at various doses (100, 200 and 400 mg/kg) was evaluated using Complete Freund's Adjuvant (CFA) in rat model. After successful induction of the arthritis, the paw oedema, hematological parameters, pro-inflammatory cytokines and arthritis marker were assessed. Also, the radiological and histopathological evaluations were also performed. There was a significant inhibition (P<0.01) of paw oedema after treatment with 100, 200 and 400 mg/kg of the methanol, ethanol and ethyl acetate Crinum pedunculatum extracts relative to the control. There was also a significant increase in haemoglobin and RBC levels as well as a decrease in WBC count. All the extracts of Crinum pedunculatum decreased IL-6 and Rheumatoid factor serum concentrations and improved joint and cartilage destruction in both contralateral and ipsilateral paw of CFAinduced rats. Significant anti-arthritic activity was observed by the 200 and 400 mg/kg ethanol and methanol Crinum pedunculatum extract. This study confirms that the bulb of Crinum pedunculatum has an anti-arthritic potential and the findings justify the folkloric use the plant as an anti-inflammatory agent.

Breast cancer is one of the most frequently observed malignancies worldwide and represents a heterogeneous group of cancers. For this reason, it is crucial to properly diagnose every single case so a specific and efficient therapy can be adjusted. One of the most critical diagnostic parameters evaluated in cancer tissue is the status of the estrogen receptor (ER) and epidermal growth factor receptor (EGFR). Interestingly, the expression of the indicated receptors may be used in a personalized therapy approach. Importantly, the promising role of phytochemicals in the modulation of pathways controlled by ER and EGFR was also demonstrated in several types of cancer. One such biologically active compound is oleanolic acid, but due to poor water solubility and cell membrane permeability that limits its use, alternative derivative compounds were developed. These are HIMOXOL and Br-HIMOLID, which were demonstrated to be capable of inducing apoptosis and autophagy or diminishing the migratory and invasive potential of breast cancer cells in vitro. In our study, we revealed that proliferation, cell cycle, apoptosis, autophagy, and also the migratory potential of HIMOXOL and Br-HIMOLID in breast cancer cells are mediated by ER (MCF7) and EGFR (MDA-MB-231) receptors. These observations make the studied compounds interesting in the context of anticancer strategies.
