Study Design Meta-analysis and systematic review. Objective This study aims to investigate and compare patient-reported outcomes, perioperative data, and complications between 2 common endoscopic approaches for lumbar disc herniation: transforaminal and interlaminar endoscopic lumbar discectomy, by published high-quality evidence. Methods Electronic databases were searched for reported outcomes following TELD and IELD. Oswestry Disability Index (ODI), visual analog scale of back (VASB) and leg pain (VASL), and MacNab criteria were evaluated. Operative time, hospital stays, blood loss, radiation exposure, and complications were investigated. Odds ratio (OR) and mean differences (MD) with 95% confidence intervals (CI) were calculated using random and fixed-effect models. Results Total of 1948 patients from 18 studies were included, consisting of 1006 patients (51.6%) in TELD group and 942 (48.4%) patients in IELD group. TELD had better improvement of postoperative ODI (MD = 1.26; 95% CI: .07-2.16; P = .04), postoperative VASB (MD = −.23; 95% CI: −.40 to −.05; P = .01) and last follow-up VASB (MD = −.25; 95% CI: −.41 to −.09; P = .002), but with longer operative time (MD = 10.1 min; 95% CI: 1.925-18.77; P < .00001) and more fluoroscopic time (SMD = 4.12; 95% CI: 3.22-5.03; P < .00001), especially in L5-S1 operation. Bed rest time was significantly longer following IELD, with no difference in VASL, hospital stays, or complication. Conclusion We found comparable clinical outcomes in terms of ODI, VAS, patient satisfaction, hospital stays, and complication between both techniques. TELD required more radiation exposure and longer operative time at L5-S1 level than IELD. Our study provided results to help consider appropriate approaches for selected patients and informed consent of benefits from each approach.

Đặt vấn đề: Thoát vị đĩa đệm thắt lưng cùng là bệnh lý cột sống thường gặp nhất, khoảng 10% bệnh nhân thoát vị đĩa đệm cần phải phẫu thuật. Mặc dù phẫu thuật mở lấy nhân đệm thoát vị đem lại hiệu quả khá tốt, các tác giả vẫn tiếp tục phát triển các kỹ thuật ít xâm lấn hơn để đem lại hiệu quả điều trị tốt nhất. Phẫu thuật nội soi đường liên bản sống là phẫu thuật xâm lấn tối thiểu có nhiều ưu điểm và được áp dụng ngày càng rộng rãi. Mục tiêu nghiên cứu: Nghiên cứu đánh giá bước đầu hiệu quả điều trị thoát vị đĩa đệm thắt lưng bằng phẫu thuật nội soi liên bản sống. Đối tượng và phương pháp: Nghiên cứu can thiệp lâm sàng không đối chứng được tiến hành tại Bệnh viện Đa khoa Quốc tế S.I.S Cần Thơ từ 7/2022 đến 10/2022. Tiêu chuẩn chọn gồm bệnh nhân bị thoát vị đĩa đệm thắt lưng cùng có triệu chứng chèn ép rễ thần kinh, không đáp ứng với điều trị nội khoa thích hợp sau 5 - 8 tuần. Kết quả: Trong 3 tháng, chúng tôi tiến hành phẫu thuật được 9 trường hợp. Tuổi trung bình 43,11 ± 5,06. Nam giới chiếm 55,56%. Toàn bộ bệnh nhân đều được chụp cộng hưởng từ, xquang động cột sống trước mổ đánh giá không trường hợp nào mất vững. Đa số các trường hợp phẫu thuật nội soi ở tầng L5S1 (66,67%), tầng L4L5 chiến 33,33%. Giá trị của VAS lưng và VAS chân giảm sau mổ có ý nghĩa thống kê với p < 0,001. Chúng tôi không ghi nhận biến chứng trong và sau mổ. Kết luận: Phẫu thuật nội soi liên bản sống là can thiệp xâm lấn tối thiểu và là phương pháp điều trị an toàn, hiệu quả trong thoát vị đĩa đệm thắt lưng cùng khi được chỉ định phù hợp.

Purpose Endoscopic spine surgery is a globally expanding technique advocated as less invasive for spinal stenosis treatment compared to the microsurgical approach. However, evidence on the efficiency of interlaminar full-endoscopic decompression (FED) vs. conventional microsurgical decompression (MSD) in patients with lumbar spinal stenosis is still scarce. We conducted a case-matched comparison for treatment success with consideration of clinical, laboratory, and radiologic predictors. Methods We included 88 consecutive patients (FED: 36/88, 40.9%; MSD: 52/88, 59.1%) presenting with lumbar central spinal stenosis. Surgery-related (operation time, complications, length of stay (LOS), American Society of Anesthesiologists Physical Status (ASA) score, C-reactive protein (CRP), white blood cell count, side of approach (unilateral/bilateral), patient-related outcome measures (PROMs) (Oswestry Disability Index (ODI), Numeric Rating Scale of Pain (NRS; leg-, back pain), EuroQol questionnaire (eQ-5D), Core Outcome Measures Index (COMI)), and radiological (dural sack cross-sectional area, Schizas score (SC), left and right lateral recess heights, and facet angles, respectively) parameters were extracted at different time points up to 1-year follow-up. The relationship of PROMs was analyzed using Spearman’s rank correlation. Surgery-related outcome parameters were correlated with patient-centered and radiological outcomes utilizing a regression model to determine predictors for propensity score matching. Results Complication (most often residual sensorimotor deficits and restenosis due to hematoma) rates were higher in the FED (33.3%) than MSD (13.5%) group (p < 0.05), while all complications in the FED group were observed within the first 20 FED patients. Operation time was higher in the FED, whereas LOS was higher in the MSD group. Age, SC, CRP revealed significant associations with PROMs. We did not observe significant differences in the endoscopic vs. microsurgical group in PROMs. The correlation between ODI and COMI was significantly high, and both were inversely correlated with eQ-5D, whereas the correlations of these PROMs with NRS findings were less pronounced. Conclusions Endoscopic treatment of lumbar spinal stenosis was similarly successful as the conventional microsurgical approach. Although FED was associated with higher complication rates in our single-center study experience, the distribution of complications indicated surgical learning curves to be the main factor of these findings. Future long-term prospective studies considering the surgical learning curve are warranted for reliable comparisons of these techniques.
