Biomarkers of ocular allergy and dry eye disease

The most common disorders of the ocular surface are dry eye disease (DED) and ocular allergy (OA). These conditions are frequently coexisting with or without a clinical overlap and can cause a severe impact on the patient’s quality of life. Therefore, it can sometimes be hard to distinguish between DED and OA because similar changes and manifestations may be present. Atopic patients can also develop DED, which can aggravate their manifestations. Moreover, patients with DED can develop ocular allergies, so these two pathological entities of the ocular surface can be considered as mutual conditions that share the same background. Nowadays, by using different techniques to collect tissue from ocular surfaces, the changes in molecular homeostasis can be detected and this can lead to a precise diagnosis. The article provides an up-to-date review of the various ocular surface biomarkers that have been identified in DED, OA, or both conditions. A alergia ocular engloba um espetro de patologias mediadas por mecanismos de hipersensibilidade imunológica e que afetam a superfície ocular. Os sintomas e sinais oculares como o prurido ou hiperemia conjuntival são muitas vezes subvalorizados e subtratados apesar do seu impacto na qualidade de vida. As diferentes entidades englobadas na alergia ocular distinguem-se pelo seu mecanismo fisiopatológico e apresentação clínica. Para o seu diagnóstico, a história clínica e o exame objetivo são fundamentais, com eventual comprovação da existência de sensibilização a alergénios, recorrendo a testes in vivo e/ou in vitro ou através de provas de provocação. O tratamento deve ser guiado pela gravidade clínica, inclui medidas não farmacológicas e farmacológicas e, em casos selecionados, imunoterapia com alergénios. O presente artigo pretende perspetivar a atual abordagem multidisciplinar do doente com alergia ocular na prática clínica, no que concerne à sua fisiopatologia, classificação, diagnóstico e opções terapêuticas.

Purpose of review Atopic dermatitis (AD) and ocular allergy aka allergic eye disease (AED) are two common conditions that often coexist in patients. However, molecular connections between these two conditions are incompletely understood. While common etiologic components including Th2 immune signaling have been suggested for AD and AED, the mechanism how current Th2-targetd therapies (dupilumab, tralokinumab) for AD can augment conjunctivitis is not well understood. Recent findings Differentially regulated genes and pathways relevant for AD disease manifestation are known. In contrast, similar information is not yet available for AED, which could be largely addressed by emerging noninvasive ocular sampling techniques. Emerging evidence indicated a reduction in goblet cell number and mucin production in a subpopulation of AD patients with AD leading to adverse ocular outcomes, while other potential mechanisms could also be involved. Involvement of particular barrier function protein(s) in AED needs further investigation. Summary Modern cytokine-targeted therapies for AD showed elevated risk for developing conjunctivitis. Recently developed noninvasive sampling techniques should be leveraged to identify AD endotypes associated with AED and with dupilumab-associated ocular outcomes.
