A retrospective review of endophthalmitis due to coagulase-negative staphylococci.

Abstract: SUMMARY We retrospectively reviewed 28 cases of postoperative endophthalmitis due to coagulase-negative staphylococci. There was an average delay between surgery and the acute presentation of 7-2 days (SD 3-3). All patients were treated with intraocular antibiotics (IOAB) or therapeutic vitrectomy with IOAB. In six of the 28 cases the organisms were resistant to gentamicin as measured by the Kirby-Bauer technique; none was resistant to cephalosporins. Isolates that had been stored by lyophilisation were recons… Show more

“…About 10% of cases are painless [21]. Consequently, the infection can run insidious, slowly progressive, and diagnosis is often delayed because of a prolonged asymptomatic interval [1,21]. Thus, despite a generally good visual outcome after treatment, nearly onethird of the cases have a final visual acuity of less than 20/400, and almost 11% have lost light perception [22].…”

“…5,21 S. warneri, S. haemolyticus, S. hominis, and S. capitis have been isolated from the eyelids and intraocular sites of patients with endophthalmitis. 15 Several studies have measured the presence of bacteria from the normal flora that entered the eye during the surgical procedure. 22 Leitch et al 8 and Bannerman et al 13 have reported that postsurgical endophthalmitis is derived from the normal flora surrounding the eye, especially coagulase-negative Staphylococcus, that gain entry into the eye during intraocular surgery.…”

“…The culture of the aqueous humor was not done. Obtaining a vitreous sample is by far the most important part, since aqueous samples have been demonstrated to be negative in up to 57% of cases in which culture positive vitreous samples were obtained [13]. If S. epidermidis over 1.0 × 10 5 /0.1 ml was injected intravitreally, spontaneous sterilization was not found [3].…”
