A retrospective multicenter analysis on redo-laparoscopic anti-reflux surgery: conservative or conversion fundoplication?

Abstract: Failure of ARS rarely occurs in the hands of experienced surgeons. Redo-ARS is feasible using a minimally invasive approach. According to our study, in terms of recurrence of symptoms, Toupet fundoplication is a superior ARS technique compared to Nissen for redo-fundoplication. Therefore, Toupet fundoplication should be considered in redo interventions for patients who initially underwent ARS with Nissen fundoplication. Furthermore, mesh repair in reoperations has a positive impact on reducing the recurrence o… Show more

“…In this study, the total recurrence rate after introduction of LF at our facility was 15% (69/457) (Figure 1), which was almost the same as the 15.5%–20% in Europe and the US 13,14 . In one study, the recurrence rate was 2.3%–42%, depending on the facility 15 .…”

“…However, they reported that the recurrence rate after MR using PTFE was 0%, and although there was a large difference compared with our 24%, the surgical procedure was performed almost in the same way. The observation period was almost the same (34 months and 30 months, respectively), although there we used the median and Dallemagne et al 14 used the average. Their diagnostic criteria for recurrent of hiatal hernia have not been described, so we could not know the details of this results.…”

“…Since 1991, when the first report of a laparoscopic fundoplication was published, laparoscopic approaches for the definitive control of GERD have evolved to the point that today they are adopted worldwide. 1,2 The continuous evolution of MIS techniques over the last few decades, together with surgeons' insatiable desire to perform more minimally invasive operations, has led to the introduction of laparoendoscopic singlesite (LESS) surgery. The first LESS operation, a tubal ligation in the 1960s, was followed much later by the consortium that defined the technique in 2008.…”
