A report on three patients with Echinococcus multilocularis: Lessons learned

Abstract: Echinococcus multilocularis (EM) is the most virulent species of the genus Echinococcus. It causes a highly lethal helminthic disease in humans. The disease may present as hepatic mass mimicking a malignant neoplasm. Due to the vascular and neural invasion, protean clinical manifestations including Budd-Chiari syndrome (BCS) may be the clinical presentation of this condition. We herein report three cases of Echinococcus multilocularis; the first case presenting as multiple hepatic space-occupying lesions, seco… Show more

“…Confusion between alveolar echinococcosis disease and neoplasia appears to be a major challenge in human medicine, in which alveolar echinococcosis may not be spontaneously included in the differential diagnoses in geographical areas where the disease is uncommon (Atanasov et al, 2013;Bansal et al, 2018). In a recent case report, hepatic E. multilocularis infection with pulmonary nodules was misdiagnosed as a cholangiocarcinoma with lung metastases in a 38-year-old woman from the Czech Republic living in Germany (Pohnan et al, 2017).…”

“…Noticeably, a paper from the UK showed an Afghan patient who migrated from Pakistan; the authors discuss the infection originating in Afghanistan [22]. There was evidence of isolated cases in India and, as far as we know, the first case was reported in Pakistan [23][24][25][26][27][28][29][30].…”

“…ELISA test (Echinococcus IgG) was positive -50 NTU (positive above 11NTU) and confirmed with positive Western-blot which revealed presence of specific for Echinococcus multilocularis IgG (7,16,18,(26)(27)(28). ELISA EM2-plus (anty-E.multilocularis) was also high positive (> 3.0ABS).…”

“…Within the liver, the parasite's unrestricted growth is responsible for focal calicifications and finally the whole necrosis. Due to vascular and neural invasion Buddi-Chiari syndrome may be the clinical presentation of this condition [26].…”
