The Brawzilian Psychiatric Reform sought to introduce new perspectives on madness, integrating other meanings about mental health; however, it seems that mad and crazy are used in everyday communications to describe social events and behaviors. This study aimed to investigate the main contents and beliefs that anchor the social representations of madness in a printed newspaper. A Thematic Content Analysis was carried out with 846 articles from the newspaper Folha de São Paulo from the years 1978 and 2018. Seven categories were constructed that anchor the idea of madness: Eccentric, Unpredictable, Intense, Irrational, Violent, Subversive, and Transgressive. The stories reinforce the stigma of madness as something divergent, strange. The construction of an inverse anchoring underlies the representations of madness, demarcating the distance between the “normal” and the “crazy. It was concluded that madness is still understood based on social devaluation and demeaning, leading to exclusionary practices.

Objetivo: Discutir sobre as condutas do Enfermeiro frente ao paciente psiquiátrico na Rede de Atenção Psicossocial (RAP), a partir da descrição dos desafios e processos de otimizações. Métodos: Foi adotado um estudo exploratório e descritivo, que utiliza como técnica a Revisão Integrativa da Literatura. A busca foi realizada nas bases de dados da Biblioteca Virtual de Saúde (BVS): SCIELO e LILACS, através de dois DeCS, cruzados com operador booleano “AND”: “Serviços de Saúde Mental” AND “Enfermagem Psiquiátrica”. Em uma primeira busca foram encontrados 852 artigos que, submetidos aos critérios pré-estabelecidos de elegibilidade e inelegibilidade, teve seu número reduzido para 15 artigos, que compõem este trabalho. Resultados e Discussões: Os principais desafios encontrados para a atuação do enfermeiro na RAP conta com a sobrecarga de trabalho deste profissional, déficit de conhecimento em como atuar na área de saúde mental, além de desafios no próprio sistema de cuidados, quanto ao espaço para desenvolver atividades com os pacientes. Já os processos de otimizações, contam com estratégias que visam o melhoramento de aceitação terapêutica como o apoio matricial, rodas de conversa e incentivos de políticas públicas que incentivam a criação de dispositivos para assistir a estes pacientes. Considerações finais: A atuação do enfermeiro no campo de saúde mental ainda enfrenta desafios quanto as suas ações e percalços dentro da RAP. Além de ser observada a necessidade do desenvolvimento de novas formas de otimizar os processos de cuidado ao paciente com transtorno mental, garantindo o que preconiza a Política Nacional de Saúde Mental.
