INTRODUCTION The knowledge of root canal anatomy is essential to ensure a successful outcome of surgical and nonsurgical root canal treatment. The aims of this article were to present 2 cases of maxillary molars with 3 mesiobuccal root canals and to review the available literature on this anatomic variation. METHODS The first case described a nonsurgical root canal treatment of tooth 16 in a 29-yearold man with the aid of a dental operating microscope. In the second case, an extracted maxillary right first molar was scanned by a micro-computed tomographic system and reconstructed 3-dimensionally using modeling software. RESULTS In both cases, the mesiobuccal root had 3 canals (type 3-2 in case I and type 3-3 in case II), whereas the distobuccal and palatal roots had a single canal. The literature review showed that the overall incidence of 3-canaled mesiobuccal roots in maxillary molars ranged from 1.3%-2.4% and that the most common root canal configuration was type 3-2. CONCLUSIONS Clinicians should always anticipate the presence of extra canals in maxillary molars and use all the available tools to locate and treat these.

K ök kanal tedavisinin amacı, tüm kök kanal sisteminin tam olarak temizlenmesi ve doldurulmasıdır. Endodontik tedavilerdeki başarı-sızlığın temel nedenleri eksik enstrümantasyon, yetersiz dezenfeksiyon, eksik kanal dolgusu ve tedavi edilmemiş kanalların varlığından kaynaklanan apikal sızıntı ve mikroorganizmalardır.1 Bu nedenle, kök kanal morfolojisinin ve muhtemel varyasyonlarının tam olarak bilinmesi kök kanal tedavisinin başarısı için oldukça önemlidir.Maksiller molar dişlerin kök kanal varyasyonları çeşitli çalışmalarda bildirilmiştir. The root canal morphology of maxillary first molars is very complex and they most commonly have three or four canals. The rate of the second mesiobuccal canal is quite high in these teeth. However, maxillary first molars are regarded as usually having a single palatal root and canal. The presence of extra palatal root in these teeth is very rare. This case report presents root canal treatment of a left maxillary first molar tooth with five canals which has two distinct palatal root and canal. Dental practitioners should always anticipate the presence of additional roots and canals in maxillary first molars, use all available tools to detect these canals. It is crucial for any clinician to have a thorough knowledge about the normal anatomy of tooth and always be vigilant for any variations to ensure endodontic success. An anatomic variant like additional canals in the palatal root of maxillary molar is not commonly reported. This clinical case report presents the application of cone beam computed tomography as a useful imaging technique in endodontics for the management of teeth with aberrant anatomy. Here, maxillary first molar having two canals in the palatal root, two canals in mesiobuccal root and one canal in distobuccal root was managed successfully using cone beam computed tomography scan as a diagnostic aid.
