Background: Meckel's diverticulum is a remnant of the omphalomesenteric duct. It can lead to intestinal perforation, obstruction and gastrointestinal bleeding. While the internal hernia caused by Meckel's diverticulum is rarely reported. Case presentation: We report a case of a 45-year old female patient who presented with intestinal obstruction and on laparotomy was found to have Meckel's diverticulum with internal hernia causing intestinal gangrene. Segmental bowel resection was performed and the patient had uneventful recovery. Conclusions: In patients with acute intestinal obstruction without previous abdominal surgery, Meckel's diverticulum and its complications should be suspected.

Цель. Представить клиническое наблюдение редкого осложнения дивертикула Меккеля (ДМ). Основное содержание. Заболевания, обусловленные наличием ДМ, служат одной из наиболее распространенных причин «острого живота» у детей: до 60 % случаев их манифестации приходится на возраст до 10 лет. Чаще всего проявлениями ДМ являются такие осложнения, как кишечная непроходимость, кровотечение, дивертикулит, грыжа Литтре. 9-летняя девочка с абдоминальной болью и рвотой, появившимися несколько часов назад, была госпитализирована в детское хирургическое отделение. Спустя несколько часов ребенок был оперирован: обнаружена странгуляционная кишечная непроходимость на мезодивертикулярной связке, соединяющей дивертикул Меккеля с корнем брыжейки тонкой кишки. Была выполнена резекция дивертикула и устранение непроходимости кишки. Ребенок выписан с выздоровлением. Заключение. Заворот кишечника вокруг мезодивертикулярной связки является одним из наиболее редких осложнений ДМ. При запоздалой диагностике данная ситуация опасна развитием грозных осложнений в виде некроза и перфорации кишечника, что делает дивертикул Меккеля одним из опасных состояний периода детства. Aim.To present of a clinical case of a rare complication of Meckel's diverticulum (MD). General fi ndings. Diseases caused by the presence of MD present one of the most common causes of an "acute abdomen" in children: up to 60% of cases of their manifestation occur in children under 10 years of age. The most common MD manifestations involve complications, such as intestinal obstruction, bleeding, diverticulitis and Litre hernia. A 9-year-old girl with abdominal pain and vomiting having appeared several hours before hospitalization was admitted to a paediatric surgical department. A few hours later, the child was operated. In the course of surgery, a strangulated intestinal obstruction was found on the mesodiverticular ligament connecting Meckel's diverticulum with the root of the small intestine mesentery. As a result, a diverticulum resection and bowel obstruction were performed. The child was discharged with recovery.Conclusions. An inversion of the intestine around the mesodiverticular ligament appears to be one of the most rare complications of MD. HighlightsMeckel’s diverticulum (MD), a remanant of omphalomesenteric duct, is the most common congenital malformation of GIT.Most patients are asymptomatic. Patients develops symptoms due to its complications like bowel obstruction, hemorrhage, diverticulitis, Littre’s hernia or perforation.Case presented with persistent features of subacute intestinal obstruction due to herniation of bowel loops through unusual mesentery of Meckel’s diverticulum extending from base of diverticulum to the band which was running from it’s tip to adjacent mesentry encircling the herniated loops of intestine forming a complete hernial sac; a rarest way of presentation.Limitation of imaging modalities in diagnosis of complications of diverticulum.Surgical intervention indicated for patients with intestinal obstruction or high risk of incarceration. The treatment option is surgical resection with diverticulectomy and reduction of internal herniation.
