A Rare Case of Submassive Pulmonary Embolism with a Right Aberrant Subclavian Artery and Thrombosed Kommerell Diverticulum

Abstract: An 81-year-old man presented with shortness of breath and was referred to our hospital with suspected acute pulmonary embolism. Enhanced computed tomography revealed a right aberrant subclavian artery with a thrombosed Kommerell diverticulum (KD), as well as deep vein thrombosis in the left leg and bilateral pulmonary artery thrombosis. Thrombosis in the KD disappeared after one month of anticoagulation treatment with rivaroxaban. Thrombosis of a KD is a rare condition that may cause distal emboli and subclavi… Show more

“…This variant is very often found by chance, since it is generally symptom free; but when it presents with dysphagia, 5 coughing, chest pain, upper limb ischemia, and subclavian steal syndrome, the suspicion is that adjacent structures such as the esophagus and trachea are subjected to compression by the artery along its abnormal course or because of aneurysmal degeneration, known as Kommerell diverticulum. 6 , 7 , 8 , 9 , 10 , 11 …”

“…When present, and depending on its diameter, a Kommerell diverticulum can compress adjacent structures and if left undiagnosed may lead to rupture of the aneurysm, dissection of the aorta, or even recurrent pneumonia. 6 , 8 , 9 There are also reports of rare cases of thrombosed Kommerell diverticula, 10 with subclavian artery stenosis, which is a condition that can result in distal emboli and subclavian steal syndrome.…”

“…Sendo muitas vezes encontrada ao acaso, geralmente não apresenta sintomatologia; porém, perante a presença de disfagia 5 , tosse, dor torácica, isquemia de membros superiores e síndrome de roubo subclávio, suspeita-se de compressão de estruturas adjacentes, como esôfago e traqueia, pela artéria em seu percurso anormal ou devido à degeneração aneurismática, denominada divertículo de Kommerell 6,7,8,9,10,11 .…”
