A Rare Case of Fatal Left Ventricular Free Wall Rupture: Case Report and Short Review

Abstract: Left ventricular free wall rupture (LVFWR) is a rare, yet lethal, complication of acute myocardial infarction (AMI), occurring in approximately 2% of cases. In the era of percutaneous coronary intervention, however, it is less frequently encountered. We were confronted with a case of AMI complicated with LVFWR after receiving thrombolytic therapy. The diagnosis was established using transthoracic echocardiography (TTE). Unfortunately, the patient passed away before surgical intervention. This case demonstrates … Show more

“…The ideal imaging modalities for separating pseudo-aneurysm from true aneurysm are CT and cardiac MRI because they offer superior spatial resolution and tissue definition to identify the myocardium from scar tissue and the pericardium. 11 Due to mechanical valve replacement, we have not done a cardiac MRI, so instead we have requested a CT angiogram to differentiate between an aneurysm and a pseudoaneurysm. Due to its high risk of catastrophic rupture, urgent surgical intervention is crucial in the treatment of this condition.…”

“…TTE offers a 100% sensitivity and 93% specificity for diagnosis 13. In the case of a diagnostic dilemma, a CT scan is useful 12. Cardiac MRI is a useful investigation to do in a non-acute setting to prepare the patient for surgery 12 14 15…”
