A rare case of Barrett’s adenocarcinoma including squamous cell carcinoma component

Abstract: We present a rare case of Barrett's cancer with a particular histology composed of various differentiated adenocarcinoma (AC) and squamous cell carcinoma (SCC) cells. A 52-year-old man with a long-standing complaint of heartburn visited a medical center for an annual check-up by esophagogastroduodenoscopy (EGD). Although high-grade reflux esophagitis with Barrett's esophagus was identified 3 years earlier, he had not visited a hospital for medication. In 2008, he underwent EGD and a 0-IIa + IIc type of superfi… Show more

“…In addition, carcinoma containing coexisting elements of infiltrating adenocarcinoma and SCC in the background of Barrett's mucosa is extremely rare. Thus far, only six cases of BAC containing elements of SCC have been reported (Table) (6)(7)(8)(9)(10)(11), making the present case the seventh case of such carcinoma. According to the guidelines for clinical and pathological studies of carcinomas of the esophagus established by the Japan Esophageal Society (4), adenosquamous carcinoma of the esophagus is defined as a lesion comprising !…”

“…It was therefore defined as BAC with an SCC component according to the Japanese classification and Barrett's adenosquamous carcinoma according to the WHO classification. Thus far, only one other case of BAC with an SCC component has been reported (11). However, that case report did not mention the ratio of the SCC component, and based on the reconstructed pictures of the report, the tumor appears to have been over 20% SCC.…”

“…Mishima et al . described an adenocarcinoma occurring in Barrett's mucosa where histological examination showed the focal squamous cell carcinoma component present within the adenocarcinoma . The Barrett's mucosa surrounding the adenocarcinoma component of the tumor displayed glandular dysplasia as a precursor to the adenocarcinoma.…”

“…Barrett´s esophagus is a well-acknowledged risk factor for esophageal AC formation; however, singular cases of SCC arising in Barrett's esophagus and/or collision tumor comprising SCC and AC elements have been documented in the literature, pointing to ambivalent carcinogenic field effects [ 1 , 2 , 3 ]. Concurrent SCC and Barrett's carcinoma has occasionally been reported in Barrett's esophagus before, however, to the best of our knowledge, not with the Barrett's segment itself [ 4 , 5 , 6 ].…”
