A Randomized, Prospective, Double-Blind, Placebo-Controlled Trial of the Effect of a Calcium Channel Blocker Ointment on Pain After Hemorrhoidectomy

Abstract: Perianal application of 2 percent diltiazem ointment after hemorrhoidectomy significantly reduces postoperative pain and is perceived as beneficial, with no increase in associated morbidity. Patients using a placebo ointment tend to take more prescription narcotics for pain relief postoperatively, with a similar usage of nonsteroidal anti-inflammatory drugs and acetaminophen, although differences were not significant.

“…In animal models, NMDA receptor antagonists have been shown to have analgesic properties in pain conditions, with similar effects also observed in humans (20,21). Conversely, calcium has been shown to be associated with the release of neurotransmitters and substances implicated in nociceptive response (22), thus the blockage of calcium channels may initiate the pain relief process (23).…”

“…In the proctology field, it is used in combination with other antibiotics against infections by anaerobic bacteria, according to the extent and severity of the injury. Considering that bacterial colonization is inevitable after hemorrhoidectomy, it is not surprising that antibiotics such as metronidazole help reduce bacterial proliferation, inflammation, and postoperative pain (18,22).…”

“…There are many approaches and treatments for postoperative pain management after hemorrhoidectomy, including local treatments such as the use of nitrates, anion exchange resins (cholestyramine), complex aluminum sucrose sulfates (sucralfate), topical anesthetics, analgesics, and calcium channel blockers (such as nifedipine and diltiazem) (8,(14)(15)(16)(17)(18)(19)(20). Metronidazole was discovered in 1950 and synthesized in 1957.…”

“…Its efficacy in reducing postoperative pain was demonstrated in two randomized trials either with respect to placebo [23] or bupivacaine HCl [24]. Topical diltiazem showed a marginal benefit in reducing pain after surgery [25][26][27]. Glyceryl trinitrate ointment improved wound healing and reduced postoperative pain [28].…”
