Connections among Intra Uterine Devices (IUD), Sexually Transmitted Diseases (STDs) and Pelvic inflammatory disease (PID) still bring doubts for health professionals; Our goal is to understand better these associations. We searched the PubMed database for articles including evidence about these topics. A common outcome found is the superiority of levonorgestrel (LNG) IUD over NOVA T (copper) device regarding clinical performance and PID rates. The modern IUDs do not elevate the risk of getting PID; however this risk is higher when a patient has a STI at the time she gets an IUD. It is wise requiring a STI test, principally for cervicitis, before placing an IUD. The epidemiologic data suggests this management mainly for woman with a new sexual partner, or with multiple sexual partners, and under age 25. It is also correct inserting an IUD and test for STI at the same day: if positive results come, antibiotics can be used. Old papers usually show a higher risk for PID in IUD users, an outcome not found in recent researches that have improved methodology and therefore are more reliable. However, the investigation concerning the STI and cervicitis remain as an important point to investigate before decision to IUD use.

Prognosis Predictors of Pelvic Inflammatory Disease among the Hospitalized Patients

Introduction: Serious reproductive health consequences linked to pelvic inflammatory disease (PID) include infertility and ectopic pregnancy. Thus, it is important to identify patients likely to have a poor prognosis in choosing the best initial treatment. The aim of our study was to identify the valuable prognosis predictors of PID among the hospitalized patients and determined cut off values of quantitative independent prognosis predictors. Material and methods:Hospital records for women hospitalized with PID were retrospectively examined. The PID patients were divided into two sub-groups according to their clinical outcome. Prognostic factors were evaluated by T-test, χ2-test, and logistic regression analysis. The cut-off values of age, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and ESR combined with CRP were calculated by receiver-operator curve analysis.Results: Independent prognosis predictors of PID were advanced age (OR=1.031, 95% CI=1.002-1.062; P=0.036), elevated ESR (OR=1.029, 95% CI=1.013-1.046; P<0.001), increased CRP (OR=1.096, 95% CI=1.027-1.169; P=0.006), and presence of endometriosis (OR=5.700, 95% CI=1.123-28.943; P=0.025). The cut-off values of age, ESR, CRP, and ESR combined with CRP were 35 years, 30.5 mm/hr, 7.0 mg/dL, and 25 mm/hr and 6.5 mg/ dL respectively. Conclusions:The initial treatment of patients who are elderly, and/or who have endometriosis, elevated ESR, or increased CRP should be carefully decided. Evaluation of ESR together with CRP is recommended to gain a more accurate prediction of disease outcome.

Univerzitet u Beogradu -Farmaceutski fakultet, Katedra za farmaceutsku tehnologiju i kozmetologiju, Vojvode Stepe 450, 11221 Beograd, Srbija Kratak sadržajUpotreba terapijskih sistema za hormonsku kontracepciju (TSHK) predstavlja značajan alternativni vid sprečavanja neplanirane trudnoće u odnosu na primenu hormonskih kontraceptiva per os, pri čemu se otklanja rizik od gastrointestinalnih neželjenih reakcija, omogućava smanjenje terapijske doze, učestalosti primene i sistemskih neželjenih dejstava i povećava nivo adherence. Do sada su razvijeni terapijski sistemi tipa transdermalnih flastera, subdermalnih (supkutanih) implanata, vaginalnih prstenova i intrauternih terapijskih (dostavnih) sistema (IUS) za hormonsku kontracepciju progestinima (progesteron, levonorgestrel, etonogestrel, nestoron) ili kombinacijama progestina i estrogena (norelgestormin + etinilestradiol, gestoden + etinilestradiol, etonogestrel + etinilestradiol, levonorgestrel + etinilestradiol, nestoron + etinilestradiol, nestoron + estradiol). U radu su prikazane glavne karakteristike navedenih tipova TSHK, kao i specifičnosti farmaceutskih proizvoda ove vrste u pogledu sastava i primene. Opisani su glavni mehanizmi za postizanje kontrolisanog produženog oslobađanja hormona i efikasnost za isporuku aktivnih supstanci kod različitih TSHK. Predstavljeni su najznačajniji savremeni pristupi u dizajnu terapijskih sistema ove vrste sa ciljem da se pojednostavi njihova primena, poboljša podnošljivost, prihvatljivost i terapijska efikasnost, a ukazano je i na mogućnost korišćenja novih tehnologija u razvoju terapijskih sistema za višenamensku prevenciju začeća i infekcije virusom humane imunodeficijencije (human immunodeficiency virus, HIV).Ključne reči: terapijski sistemi za hormonsku kontracepciju; transdermalni flaster; subdermalni (supkutani) implant; vaginalni prsten; intrauterini terapijski (dostavni) sistem; progestini; estrogeni; kontrolisano/produženo oslobađanje; tehnologije za višenamensku prevenciju.
