Objective: To evaluate the efficacy of videotaped instruction of a behavioral intervention to reduce crying among newborns.Methods: Mothers of healthy, full-term newborns were recruited from the postpartum unit of a large community hospital for a prospective, randomized, controlled trial of an intervention to reduce infant crying. Mothers participating in the intervention viewed a videotape with instructions involving swaddling, side positioning, white noise, jiggling, and sucking. Mothers in the control group viewed a videotape with instructions for normal newborn care. Intervention was assessed by mean hours per day of infant total crying (fussing, crying, and unsoothable crying) and sleeping as recorded in a diary 3 days a week during the 1st, 4th, 6th, 8th, and 12th weeks of age; the Parenting Stress Index was also used during the 6th and 12th weeks.Results: Fifty-one mother-infant pairs were recruited; 35 completed the study (18 intervention and 17 controls). Sixteen were lost to follow-up. There were no statistically significant differences between the groups in the hours of mean daily total crying or sleeping during the 1st, 4th, 6th, 8th, or 12th weeks of age. For example, during the 6th week of age mean daily total crying was 1.9 hours for infants in the control group versus 2.2 hours for infants in the intervention group (P ‫؍‬ .4); sleep was 14.5 hours for infants in the control group versus 14.4 hours for infants in the intervention group (P ‫؍‬ .8). During the 12th week mean daily total crying was 1.2 hours for infants in the control group versus 1.8 hours for infants in the intervention group (P ‫؍‬ .8) and sleep was 14.1 hours for infants in the control group versus 14.0 hours for infants in the intervention group (P ‫؍‬ 1.0). There was no difference between the groups in the Parenting Stress Index during the 6th week of age.

Главной причиной преждевременного завершения грудного вскармливания, по данным литературы, является гипогалак-тия, ее профилактика и лечение могут значительно продлить кормление грудью. В статье представлен опыт профилактики гипогалактии путем чтения лекций на курсах для беременных женщин в рамках антенатальной подготовки к родам, что привело к достоверным различиям в продолжительности грудного вскармливания: до 1,5 года кормили грудью 62% про-слушавших лекции женщин и только 37% женщин, которые не посещали курсы по собственной инициативе (р < 0,001). Таким образом, антенатальная профилактика гипогалактии, информирование беременных о рекомендациях ВОЗ (частое прикладывание ребенка к груди, контакт «кожа к коже», ночные кормления и сцеживание) могут значительно продлить грудное вскармливание. Кроме того, может быть полезно применение лактогонных средств. В статье дан обзор современ-ной информации об эффективности и применении в разных странах лактогонных средств, как травяных, так и фармако-логических. Особое внимание уделено травам: фенхелю, анису, крапиве и тмину, входящим в состав травяного чая Лактафитол®, широко применяемого в России.

Infantile colic is a self-limiting condition that occurs in infants. It is a common disturbance occurring in the first three months of life but is generally harmless. The exact aetiology is unknown, but it is associated with a vicious cycle of the infant crying and swallowing of air. Mothers use a number of methods to relieve their infants from this condition. However, there is limited evidence that support the use of complementary and alternative treatments (such as herbal supplements, and a manipulative approach) or behavioural interventions. The impetus of this paper is thus to explore local or alternative methods of the management of infantile colic. A scoping review of existing literature was done to assess all research on local practices of infantile colic management by nurses and doctors in developed countries and work carried out in Africa. This was done to identify what has been done and to identify any gaps in research. The majority of the studies reviewed revealed positive effects of unorthodox or local methods in managing infantile colic although some cause complications for the infants. There are very few published articles on this subject and methods used. The researchers, therefore, recommend more controlled trials to test the effectiveness of the different local substances used to manage infantile colic.
