El dolor de miembro fantasma es una complicación frecuente tras la amputación de un miembro. Su fisiopatología es compleja y el tratamiento farmacológico disponible no es satisfactorio. Por ello, el foco de atención se está desviando hacia terapias no farmacológicas como la terapia de espejo, una técnica descrita en los años 90 por Ramachandran y que consiste en crear la ilusión de estar viendo el miembro amputado mediante la observación del reflejo del otro miembro en un espejo. Un estudio de 2007 concluía que es eficaz para el tratamiento del dolor fantasma de amputados del miembro inferior. El objetivo principal de este trabajo fue revisar la literatura científica más reciente sobre la aplicación de la terapia de espejo para el tratamiento del dolor fantasma de amputados. Como objetivos específicos, valorar la evidencia científica que avala su uso; identificar contraindicaciones y efectos adversos; analizar los protocolos de aplicación empleados, y averiguar si hay factores individuales susceptibles de influir en la respuesta de los pacientes.Mediante una búsqueda bibliográfica en Medline, Embase Cochrane Library y Cuiden se localizaron y evaluaron dos revisiones sistemáticas y ocho estudios primarios.La terapia de espejo es una opción terapéutica económica, de fácil aplicación, con pocas contraindicaciones y que no suele producir efectos adversos severos, pero no se ha encontrado suficiente evidencia científica que avale su uso en amputados. Hay muchas dudas con respecto a su mecanismo de acción, forma idónea de aplicación y eficacia. Es necesario realizar más estudios.

ZusammenfassungÜber die Hälfte der Patienten nach einer Amputation leidet an Phantomschmerzen, deren Entstehung bislang nicht vollständig geklärt ist. Seit 2019 existiert eine aktualisierte S2k-Leitlinie zur Behandlung neuropathischer Schmerzen 1. Hier wird die Behandlung von Phantomschmerzen mit einem Verweis auf die unzureichende Datenlage nachrangig erwähnt. Die Spiegeltherapie ist eine verbreitet angewandte Behandlung im Rahmen eines multimodalen Konzepts. Zur Klärung der Evidenz für deren Wirksamkeit wurde in drei Datenbanken ausschließlich nach randomisierten klinischen Studien (RCT) gesucht, die die Spiegeltherapie mit einer Kontrollgruppe oder anderen Therapieformen verglichen. Die Schmerzreduktion wurde anhand der visuellen Analogskala, die Studienqualität mithilfe der PEDro-Skala bewertet.Von den sechs in die Analyse eingeschlossenen Studien wiesen fünf einen positiven Effekt der Spiegeltherapie bezüglich der Schmerzstärke und eine Studie eine reduzierte Häufigkeit von Schmerzepisoden nach.Somit besteht moderate Evidenz für die Reduktion von Phantomschmerzen nach Amputation durch die Spiegeltherapie. Zur Bestätigung sind weitere qualitative Studien mit aussagekräftigen Fallzahlen notwendig.

Background and Objective: Phantom limb pain (PLP) concerns >50% of amputees and has a negative impact on their rehabilitation, mental health, and quality of life. Mirror therapy (MT) is a promising strategy, but its effectiveness remains controversial. We performed a systematic review to: (i) evaluate the effectiveness of MT versus placebo in reducing PLP, and (ii) determine MT effect on disability and quality of life.Databases and Data Treatment: We selected randomized controlled trials in five databases (Medline, Cochrane Library, CINAHL, PEDro, and Embase) that included patients with unilateral lower or upper limb amputation and PLP and that compared the effects on PLP of MT versus a placebo technique. The primary outcome was PLP intensity changes and the secondary outcomes were PLP duration, frequency, patients' disability and quality of life.Results: Among the five studies included, only one reported a significant difference between the MT group and control group, with a positive MT effect at week 4. Only one study assessed MT effect on disability, and found a significant improvement in the MT group at week 10 and month 6. Conclusions: Our systematic review did not allow concluding that MT reduces PLP and disability in amputees. This lack of strong evidence is probably due to (i) the low methodological quality of the included studies, and (ii) the lack of statistical power. Future trials should include a higher number of patients, increase the number and frequency of MT sessions, have a long-term follow-up, and improve the methodological quality. SIGNIFICANCERecent meta-analyses concluded that mirror therapy is effective for reducing phantom limb pain. Conversely, the present systematic review that included only studies with the best level of evidence did not find any evidence about its effectiveness for this condition. We identified many ways to improve future randomized controlled trials on this topic: increasing the number of participants, reducing the intra-group heterogeneity, using a suitable placebo, and intensifying the mirror therapy sessions and frequency.
