Artéria radial versus veia safena para revascularização do miocárdio: metanálise (não houve diferença estatisticamente significante)Radial artery versus saphenous vein to myocardial revascularization: meta-analysis (there is no statistically significant difference) AbstractObjective: To compare the effectiveness of the radial artery graft with the saphenous vein graft for myocardial revascularization in association with the internal thoracic artery.Methods: A systematic review of the literature was made, searching for articles in the MEDLINE and LILACS databases. Two independent researchers performed the selection of articles, carefully evaluating the methodology described in articles considered relevant to the subject. Only controlled trials with an adequate randomization system were included. In all situations, when the researchers did not reach an agreement, a consensus meeting was held. No restrictions were stipulated in respect to the follow-up period for the angiographic evaluation of the graft, the vessel treated or the characteristics of the patients included in the studies. The results are expressed as Relative Risks (RR), with 95% Confidence Intervals (CI) to compare the effectiveness between the radial artery and the saphenous vein.Results: Based on these criteria, three studies were included. We did not find statistical differences between the patencies of the grafts (RR 0.53 [95% IC 0. -2.18]).Conclusion: Despite the studies having good methodologies we did not observe statistically significant differences between the grafts. The statistical power of the meta-analysis is low and therefore, more randomized controlled trials are necessary, with adequate sample sizes to detect possible differences between the different treatments.Descriptors: Radial artery. The burden of diseases associated with diabetes mellitus is dramatic: adults with diabetes mellitus are 2 to 4 times more likely to have cardiovascular diseases than those without it, and at least 65% will die because of diabetes complications. The revascularization strategy in these types of patients included percutaneous coronary interventions with bare metal stents or medicated stents and surgical coronary artery bypass grafting (CABG), but it is well known that in the diabetic patient with two or more vessel disease, the surgical strategy allows the best mid-and longterm results. Moreover, benefits of CABG surgery are limited by life expectancy of the most common type of graft, the saphenous vein (SV). Nearly 40 years after the introduction of bypass surgery, the rate of vein graft failure remains at high levels. Several arterial conduits had been studied as alternative conduits to SV: the Right Internal Thoracic Artery (RITA), the Radial Artery (RA), the Gastroepiploic Artery (GEA) and the Inferior Epigastric Artery (IEA), 40 years ago. The aim of our article is to review the scientific literature of the past 15 years to answer this question: are we ready to treat the diabetic patient, with a completely arterial revascularization, avoiding the use of the great saphenous vein grafts?

Širdies, krūtinės ir kraujagyslių chirurgijos klinika Raktažodžiai 91,0, 89,9, 90,1 ir 75,0 proc. atvejų, o veninėse -79,9, 68,6, 57,4 ir 51,0 proc., atitinkamai. Išvados ĮvadasLigonių, kuriems atlikta miokardo revaskulizacijos operacija, tiek gyvenimo trukmė, tiek jo kokybė tiesiogiai priklauso nuo operacijos metu suformuotų vainikinių jungčių funkcionavimo. JAV kasmet atliekama daugiau kaip 350 tūkst. miokardo revaskulizacijos operacijų (1), Lietuvoje 2003Lietuvoje -2004Lietuvoje m. atlikta iki 1800Lietuvoje -2000. Dauguma tyrimų, kurių metu tirtas jungčių pratekamumas, baigties tašku autoriai nurodo laiko trukmę nuo buvusios operacijos iki pooperacinės koronarografijos. Be to, dažniausiai įrodo-mas arterinių jungčių pranašumas lyginant su veninėmis. Arterinė jungtis, panaudojant kairiąją vidinę krūtinės arteriją (KVAK), dažniausiai suformuojama su priekine tarpskilveline šaka (PTŠ) ir (ar) su jos pirmąja diagonaline šaka, o veninės jungtys -su likusiomis vainikinėmis arterijomis. Pateikiamas 40-50 proc. veninių jungčių praeinamumas po 10 metų, o arterinių -90-95 proc. (4-7). Tačiau pastaraisiais metais pasirodo vis daugiau straipsnių, kuriuose pabrėžiama mažo laipsnio susiaurėjimo natyvinėse arterijose reikšmė arterinių jungčių užsidarymui (8, 9).Šio tyrimo tikslas -ne tik palyginti veninių ir arterinių jungčių išgyvenimą, bet ir jų funkcionavimą skirtingose miokardą maitinančių vainikinių arterijų zonose. Jungčių rūšys (arterinė ar veninė) bei jų suformavimo vieta pateikiama 2 lentelėje. Kairioji vidinė krūtinės arterija buvo panaudota priekinės tarpskilvelinės šakos revaskulizacijai, kaip viena ar sekvencinė jungtis su pirmąja diagonaline šaka. Kairioji vidinė krūtinės arterija panaudota 520 (75,9 proc.) ligonių, iš kurių penkiems suformuota po dvi jungtis, dešinioji -12 ligonių, o prieskrandžio -vienam ligoniui. Likusios 1644 jungtys suformuotos panaudojant kojos didžiąją poodžio veną. Jungtis su priekine tarpskilveline šaka atlikta 106 (6,4 proc.) ligoniams, su pirmąja diagonaline šaka -364 (22,1 proc.), su gaubiamąja šaka -629 (38,3 proc.), o su dešiniąja vainikine arterija -545 (33,1 proc.) ligoniams. Tirtųjų kontingentas ir tyrimo metodika Statistinė analizėDuomenys vertinti statistikos programa "SPSS v.12 for Windows". Statistiniam analizuojamų kintamųjų palyginimui taikytas Stjudento (t) kriterijus. Duomenys pateikiami kaip aritmetinis vidurkis plius/minus standartinis nuokrypis (M±SD). Skirtumas statistiškai reikšmingas, kai p<0,05. Nagrinėjant rezultatus, jungties išgyvenimas apskaičiuotas pagal Kaplan-Meier išgyvenimo kreivę. Rezultatai ir jų aptarimasAnksčiau mūsų publikuotame straipsnyje buvo teigiama, kad koronarografijos, atliktos daugiau kaip po 120 mėn., metu arterinėse jungtyse geras pratekamumas rastas net 76,5 proc. ligonių, o veninėse -tik 34,6 proc. (3). Šio tyrimo metu mes pastebėjome geresnį tiek arterinių, tiek veninių jungčių išgyvenimą (1 pav.).
