A randomised controlled trial of cognitive behaviour therapy vs treatment as usual in the treatment of mild to moderate late life depression

Abstract: CBT alone and TAU alone produced significant reductions in depressive symptoms at the end of treatment and at 6 months follow-up. CBT on its own is shown to be an effective treatment procedure for mild to moderate late life depression and has utility as a treatment alternative for older people who cannot or will not tolerate physical treatment approaches for depression.

“…I do not think psychotherapy is a panacea, but evidence supports the efficacy of certain forms of manualized psychotherapies for mild-to-moderate depression. [6][7][8] Although I identify myself as a psychopharmacologist, I think it is better to start with psychotherapy rather than drugs for those conditions. But the reality of health care is that psychotherapy is relatively more expensive than medication.…”

“…However, a fundamental observation can be made about psychotherapy now-and that is that, although certain psychotherapies [6][7][8] have been shown to be beneficial in depression, most people do not receive the types with proven efficacy.…”

“…Two recent randomized controlled trials conducted in the UK and recruiting from primary care [9,10] have tended to correct these errors by recruiting older samples with mean ages in the mid-70s, utilising intention to treat designs. Nevertheless, there remains a need for more robust treatment trials examining CBT as an efficacious treatment for late life depression, and most especially with the oldest-old given the demographic transition evident across the globe.…”
