A randomised controlled trial comparing bipolar hemiarthroplasty with total hip replacement for displaced intracapsular fractures of the femoral neck in elderly patients

Abstract: The best treatment for the active and lucid elderly patient with a displaced intracapsular fracture of the femoral neck is still controversial. Randomised controlled trials have shown that a primary total hip replacement is superior to internal fixation as regards the need for secondary surgery, hip function and health-related quality of life. Despite good results achieved with total hip replacement in this group, most orthopaedic surgeons still advocate hemiarthroplasty for this injury. We studied 120 patient… Show more

“…In contrast, one recent randomized trial comparing an arthroplasty using a cemented Exeter TM bipolar implant (Stryker UK Ltd, Newbury, UK) with THA showed excellent functional outcomes in both groups [4]. Although the HHS was higher in the THA group, there was no difference in the ADL or EQ-5D scores [4]. Although these trials are not comparable, they strongly indicate different results in different types of hemiarthroplasties.…”

“…Even so, it still is being defended as a treatment option for frail elderly patients [27]. “…Two other studies also suggest better outcomes with THA as compared with HA [8,18]. In a recent randomized trial, Blomfeldt et al studied elderly patients with acute displaced intracapsular fracture of the femoral neck and concluded total hip replacement provided better function than a bipolar HA, without increasing the complication rate [6]. However, these studies have methodologic limitations such as small sample size, short duration of followup, inadequate information on randomization technique, and limited number of outcomes that were compared.…”
