A quality improvement project to improve the identification and management of delirium

Abstract: Background: Efforts to reduce delirium burden through screening, identification, and prevention is considered one of the major public health priorities of the last decade. In 2017, an audit of delirium screening in our unit revealed suboptimum assessment of our patients, with compliance with the Confusion Assessment Method for the ICU (CAM-ICU) assessments highly variable and ad hoc, and sometimes not at all. A separate sedation audit also revealed that our sedation practices did not align with current critical… Show more

“…Impact of the CAM-ICU tool on delirium detection : The compliance rate for performing CAM-ICU during the intervention period was 98.9%. The nurses received proper training to complete the CAM-ICU tool every 12 hr; this task contributed to a compliance rate of more than 98%, consistent with published studies (Blevins & DeGennaro, 2018; Sutton-Smith, 2021). A total of 28.6% ( n = 108) of CCU patients screened positive for delirium at least once during CCU admission.…”

“…It has undergone extensive development, validation and is routinely utilised with a once-daily assessment 5 18. However, in a previous study, the nurse-to-intubated-and-ventilated-patient ratio was high (1:1), and the adherence to the CAM ICU assessment was found to be low at 45% 19. Consequently, to ensure better compliance with the PODIUM protocol, it is advisable to initiate the CAM ICU assessment at a low frequency.…”

Implementing an hourly assessment could potentially result in a decreased rate of compliance.…”

“…Sutton‐Smith 18 takes us to a 24‐bed general ICU in New Zealand and reports about the planning, implementation and outcome evaluation of a quality improvement initiative related to delirium and sedation assessment and management. The project was based on the Plan‐Do‐Study‐Act (PDSA) framework 19 and involved the development of a delirium clinical pathway and sedation algorithm, a delirium resource book, a family information pamphlet and an online education module, supplemented by extensive teaching, newsletter reminders and posters in key areas.…”
