A Quality-Based Review of Randomized Controlled Trials of Psychodynamic Psychotherapy

Abstract: Existing randomized controlled trials of psychodynamic psychotherapy are promising but mostly show superiority of psychodynamic psychotherapy to an inactive comparator. This would be sufficient to make psychodynamic psychotherapy an "empirically validated" treatment (per American Psychological Association Division 12 standards) only if further randomized controlled trials of adequate quality and sample size replicated findings of existing positive trials for specific disorders. We do not yet know what will eme… Show more

“…First, the efficacy of psychodynamic therapy remains controversial because of a lack of adequately conducted trials (7)(8)(9)(10). Second, while different psychotherapeutic approaches for depression are considered equally efficacious by many (11), high-quality studies directly comparing psychotherapies for depression are rare (12), and the literature is dominated by superiority trials designed to show significant differences between conditions, which cannot demonstrate equal efficacy (13).…”

“…First, the psychoanalytic community as a whole has been historically disinterested or resistant to the value of research, which has resulted in the critical scientific evaluation of psychodynamic treatments lagging behind the evaluation of other forms of psychiatric and psychological interven tions (Gerber 2010 Third, many of these studies have focused on brief psychodynamic treatments, whereas many psychodynamic clinicians are interested in elucidating the mechanisms of change of longer term treatments which aim at deeper structural changes in the patient's personality organisation rather than solely symptom improvement.…”

“…We also aimed to evaluate the quality of the studies identified by carrying out a qualitative review, and using the Randomized Controlled Trial Psychodynamic Quality Rating Scale (RCT-PQRS) 28 for RCTs and quasirandomized studies.…”

“…Similarly, standards for reviewing these studies are often not as transparent as they could be, giving rise to further heterogeneity. To meet this challenge, in this review the quality of the studies was judged using the Randomized Controlled Trial Psychotherapy Quality Rating Scale (RCT-PQRS) 28 . overall quality of the study.…”
