Accumulating evidence supports the reliability and validity of the diagnosis of personality disorders (PDs) in adolescents, but whether the current Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM–5), criteria are optimal to capture and help detect emerging PDs in this age-group remains controversial. The Level of Personality Functioning Scale (LPFS), included in the alternative model for personality disorders, may provide a more developmentally sensitive way to identify impaired personality features in young people. This study investigates the feasibility of the LPFS in adolescents by examining the psychometric properties of the Semistructured Interview for Personality Functioning DSM–5 in a clinical sample of referred adolescents (N = 84) and in a community sample (N = 12). Additionally, referred adolescents completed self-report questionnaires pertaining to symptom severity, personality functioning, and personality traits. In general, good interrater reliability and internal consistency were observed, and the associations with external variables largely followed theoretical prediction. Interestingly, and in contrast to data on adults, we found no significant associations between the LPFS scores on the one hand and traditional DSM–5 PD diagnoses in the clinical sample on the other (except for borderline PD criteria). In discussing these findings, we argue that the assessment of personality functioning may be better suited for detecting personality pathology in adolescence than the traditional Section II criteria.

The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) recognizes a developmental perspective on personality pathology owing to its proposal to conceptualize personality pathology in terms of maladaptive personality traits. Previous research has found that the DSM-5 maladaptive traits and the five-factor model (FFM) for normative personality traits share common underlying dimensions. Although the DSM-5 generally assumes DSM-5 traits to be extreme versions of FFM traits, empirical evidence is scarce in adolescents. The present study therefore extended previous studies by comparing the Personality Inventory for DSM-5 and the Big Five Inventory-2 (BFI-2) in an adolescent sample (n ϭ 353), using item response theory. Results indicated an underlying dimension for all domain pairs except for FFM Openness and Psychoticism. Consistent with the general assumption, IRT results demonstrated that Personality Inventory for DSM-5 scales generally provided more information than the BFI-2 scales at the upper levels of the latent dimension. The BFI-2 scales provided more information at the lower levels. For FFM Conscientiousness and Disinhibition, however, the BFI-2 scale provided more information for almost the entire range of the latent dimension. The findings indicate similarities in the DSM-5 conceptualization of personality pathology between adults and adolescents and at the same time identify important differences that need to be considered.

IntroductionWe propose that a dimensional, multilayered perspective is well suited to study maladaptive personality development in youth. Such a perspective can help understand pathways to personality pathology and contribute to its early detection. The research project ‘APOLO’ (a Dutch language acronym for Adolescents and their Personality Development: a Longitudinal Study) is designed based on McAdams’ integrative three-layered model of personality development and assesses the interaction between dispositional traits, characteristic adaptations, the narrative identity and functioning.Methods and analysisAPOLO is a longitudinal research project that takes place in two outpatient mental healthcare centres. Participants are youth between 12 years and 23 years and their parents. Data collection is set up to build a data set for scientific research, as well as to use the data for diagnostic assessment and systematic treatment evaluation of individual patients. Measurements are conducted half-yearly for a period of 3 years and consist of self-report and informant-report questionnaires and a semistructured interview. The included constructs fit the dimensional model of personality development: maladaptive personality traits (dispositional traits), social relations, stressful life events (characteristic adaptations), a turning point (narrative identity) and functioning (eg, achievement of youth specific milestones). Primary research questions will be analysed using structural equation modelling.Ethics and disseminationThe results will contribute to our understanding of (the development of) personality pathology as a complex phenomenon in which both structural personality characteristics as well as unique individual adaptations and experiences play a role. Furthermore, results will give directions for early detection and timely interventions. This study has been approved by the ethical review committee of the Utrecht University Faculty for Social and Behavioural Sciences (FETC17-092). Data distribution will be anonymous and results will be disseminated via communication canals appropriate for diverse audiences. This includes both clinical and scientific conferences, papers published in national and international peer-reviewed journals and (social) media platforms.
