A prospective multi-institutional validity study to evaluate the accuracy of clinical diagnosis of pathological stage III gastric cancer (JCOG1302A)

Abstract: Clinical diagnosis of T3/T4 tumors was not an optimal criterion to select patients for intensive NAC trials because more than 10% of patients with pathological stage I disease were included. We propose the criterion "cT3/T4 and cN1/N2/N3" instead.

“…They reported accuracy for T and N staging by SL. Preoperative T and N staging has a solid limitation due to the discrepancy between clinical and pathological diagnosis, and SL could not solve this problem. Not only that, SL has a potential disadvantage for T and N staging different from imaging, because it cannot provide complete exploration of the primary lesion and regional lymph nodes.…”

“…Epidemiological investigation has demonstrated that the highest morbidity and mortality associated with gastric cancer is found in Asia, especially in China, as the second lethal malignancy. Despite tremendous advances made in both diagnosis and therapeutics over the last decade, the 5‐year survival rate of gastric cancer is still far from satisfactory, which is principally a result of the occurrence of distal metastasis . Meanwhile, sensitive and reliable early diagnostic technology is still missing in clinical practice, which consequently associates with the clinically high frequency of an advanced stage of gastric cancer when pathologically confirmed .…”

The mainstay therapeutic options for this disease include partial surgical resection, chemotherapy and targeted therapies .…”

“…The previous subjective comparisons reflect a particular characteristic, and it is uncertain depending on the subject group. Although the method based on pathological diagnosis is the best standard [14], it costs a lot of time and labor work while it is unreviewable. Therefore, existing methods are not suitable for the generalized research on colorectal images in the testing phase of medical devices.…”
