A prospective microbiologic surveillance program to detect and prevent the transfusion of bacterially contaminated platelets

Abstract: After two patients received bacterially contaminated platelet transfusions, a prospective surveillance program was instituted to perform Gram staining and microbiologic culturing of platelets at the time of transfusion. In 12 months, 3141 random-donor platelet pools (prepared from 14,481 units) and 2476 single-donor apheresis units were cultured. All single-donor apheresis units were sterile, but 6 (0.19%) of the random-donor pools were found to be bacterially contaminated, with 1 unit of 5 in the pool being t… Show more

“…One in 2000 units of blood is contaminated from skin or blood at the time that it is drawn [23,24]. Despite leucoreduction and cold storage, about 1 in 30 000 stored red blood cell units can be demonstrated at some point to be bacterially contaminated.…”

“…Currently, risk of infection associated with PRBC transfusion is 1:2,000 for bacterial infection; 1:58,000-149,000 for hepatitis B; and 1:872,000-1,700,000 for hepatitis C [23].…”

Analyzing these data, we may consider Group 2 as possibly more severe than Group 1.…”

“…There are several factors that playa role in the incidence and detection of Tx sepsis, such as the storage temperature ( below O°C, 4°C or 22°C), the volume of blood cultured, the temperature and the culture media used for the detection, and the sensitivity of the method. Nevertheless, the incidence of contaminated products ranges from 1: 13,100 to I :330 for red blood cells, from 1:20,000 to 1:3,600 for single donor apheresis platelets, and from 1: 10,000 to 1:250 for pooled random platelet concentrates (6,7,8,9,10).…”

“…However, false positive and false negative results are encountered. The method is laborious, expensive and its efficiency is still debatable (9), though a prospective study showed evidence that it might be suitable for routine screening (10). Bacterial cultures: different results have been reported since there is no standardization as to the volume to be inoculated, the culture medium, temperature, atmosphere, time of incubation and microorganism quantification.…”
