Chimeric antibodies have been produced against a pancarcinomic tumor associated antigen, TAG-72, by fusing the genes for the variable region of mouse MAb B72.3 to the genes for the constant region of human IgG. In our efforts to optimize the pharmacokinetics of plasma clearance and the efficiency of tumor localization and penetrance of cB72.3, we have now developed truncated versions of immunoglobulin heavy chains. The domain-deleted antibodies are produced by transfecting cells that produce chimeric kappa chains with expression vectors that encode chimeric heavy chains lacking the sequences that encode the CH2 domain, CH3 domain, or both. Despite the absence of these domains, the transfectomas secrete H2L2 tetramers with appropriate antigenic specificity. All the domain-deleted immunoglobulins can be purified by chromatography on Protein G Sepharose which binds to a site on the Fab region that is retained in the domain-deleted antibodies. The CH2CH3 domain-deleted immunoglobulin produced in cell culture is analogous in size to enzymatically produced F(ab')2.

The recently developed technetium-99m-labelled monoclonal antibody-170 (MAb-170) was designed for diagnostic use in patients suffering from gynaecological adenocarcinoma. Following in vitro studies which showed immunoreactivity of this antibody to more than 90% of human adenocarcinomas, the present investigation was initiated to verify its usefulness for radioimmunoscintigraphy of ovarian tumours. Most of the 30 patients participating in this study underwent immunoscintigraphy prior to first-look surgery. Biokinetic evaluation in two patients showed a plasma half-time of 18.9 h (mean value, n = 2, r = 0.98) and a biexponential total body curve with values of 7.7 h and 17 days (r = 0.98). The mean 24-h urinary excretion was 12% of the injected dose. Radioimmunoscintigraphy using the MAb-170 recognised 12 of 13 cases of adenocarcinoma of the ovaries, corresponding to an overall sensitivity of 92.3%. Specificity was 94.1% (16/17). The calculation of accuracy yielded a figure of 93.3% (28/30). Of 33 known lesions, 26 were visualised successfully; thus the locoregional sensitivity was 78.8%. Of 29 benign tumour sites, 28 showed no evidence of tracer accumulation, corresponding to a locoregional specificity of 96.6%. The smallest lesion visualised was an adenocarcinoma of the corpus uteri with a diameter of 1.5 cm. Technetium-99m labelled MAb-170 is a promising new radiopharmaceutical for immunoscintigraphy of ovarian adenocarcinoma.

We used radiolabelled monoclonal antibodies (MoAbs) to prove disease persistence after treatment for ovarian carcinoma. Twelve patients with histologically confirmed ovarian carcinoma were studied. They received 5 mCi (1 mg) of iodine-131-B72.3 by intravenous injection before and after a complete course of chemotherapy. Images were obtained with a LFOV gamma camera 2 h after MoAb administration and daily up to 6 days. Before treatment 8 patients had a true positive scan. Questionable antibody uptake was observed in 2 patients while 1 had a true negative scan and 1 a false-negative examination. After treatment the therapeutic response was evaluated. Five patients had partial remission and antibody scan showed persistence of disease in all patients except 1. Four patients showed progression of the disease and 1 no change. The antibody scan was positive in 4 and questionable in 1. Two patients had complete remission and negative antibody scans. Computed tomography (CT) could not always discriminate postoperative fibrosis from tumour lesions, especially when the peritoneum was involved in the disease. High serum levels of tumour markers were constantly associated with the presence of tumour lesions, but normal values did not guarantee absence of disease. We conclude than the antibody scan is complementary to CT and serum tumor markers in the definition of therapeutic response.
