ObjectivesHigh crown‐to‐implant ratios may lead to complications due to unfavorable occlusal forces, including nonaxial forces, on the bone surrounding the neck of the implant and within the connection of the crown and implant itself. The aim of this study was to perform a systematic review on the influence of crown‐to‐implant ratio of single‐tooth, nonsplinted, implants on biological and technical complications.Materials and Methods MEDLINE (1950–January 2018), EMBASE (1966–January 2018), and Cochrane Central Register of Controlled Trials database (1800–January 2018) were searched to identify eligible studies. Inclusion criteria were as follows: crown‐to‐implant ratio of single‐tooth, nonsplinted, implant‐supported restorations in the posterior maxilla or mandible and follow‐up of at least 1 year. Main outcome measures were as follows: implant survival rate, marginal bone level changes, biological complications, and technical complications. Two reviewers independently assessed the articles. A meta‐analysis was carried out for implant survival rate and peri‐implant bone changes.ResultsOf 154 primarily selected articles, eight studies fulfilled the inclusion criteria. Study groups presented a mean crown‐to‐implant ratio varying from 0.86 (with 10‐mm implants) to 2.14 (with 6‐mm implants). The meta‐analysis showed an implant survival of more than 99% per year and mean peri‐implant bone changes of <0.1 mm per year. Limited biological and technical complications were reported.ConclusionData reviewed in the current manuscript on crown‐to‐implant ratio, ranging from 0.86 to 2.14, of single‐tooth, nonsplinted, implants did not demonstrate a high occurrence of biological or technical complications.

Importance Patellofemoral osteoarthritis (PFOA) is a common problem in the active population, affecting 2-11% of men and 8-24% of women over 55 years of age. Conservative measures are often ineffective, and patellofemoral arthroplasty (PFA) is an appealing solution considering that it is minimally invasive procedure, without burning any bridges if conversion to a total knee arthroplasty (TKA) is required. Objective This work aims to systematically review the current available literature on outcomes from PFA or TKA for isolated PFOA. In particular, the research questions are: (1) Is the revision surgery rate higher in PFA than in TKA? (2) Is there any difference in conversion to TKA or reoperation rates between inlay and onlay implant design? (3) What influence has tibiofemoral osteoarthritis (TFOA) progression in PFA complication rate? (4) How many PFAs are revised or converted to TKA because of TFOA progression? Evidence review A comprehensive search was performed through PubMed, Cochrane Library, Scopus, CINAHL and Google Scholar databases to search for relevant studies on PFA. Inclusion and exclusion criteria were defined. A 20-item methodological quality tool for assessing case series studies was used. Findings Thirty-seven papers were found to be case series, and one was a case-control study (PFA vs TKA). The general methodology level was weak. However, PFAs have a statistically significant higher complication and consequently reoperation rate, with a statistically significant lower survivorship rate compared to TKA for PFOA. When taking into consideration PFA implant design, those numbers were different. Newer PFA designs, contrary to old ones, showed comparable complication and revision rates to those of TKA. However, newer PFAs' survivorship is still significantly lower than TKAs' one. Although TFOA progression is not necessarily a complication, it represents up to one-third of the reasons for PFA requiring revision, playing a major role in implant's survivorship. Conclusions and relevance There is a lack of highlevel evidence on PFA. However, PFAs with newer implant design showed to be as reliable in terms of the complication and reoperation rate as TKA. Progression of TFOA still remains an issue, representing up to one-third of the overall complications. Level of evidence III, systematic review of level IV and V studies.

Promising efficacy results of chimeric antigen receptor (CAR) T-cell therapy have been tempered by safety considerations. Our objective was to comprehensively summarize the efficacy and safety of CAR-T cell therapy in patients with relapsed or refractory hematologic or solid malignancies. MEDLINE, Embase, and the Cochrane Register of Controlled Trials (inception -November 21, 2017). Interventional studies investigating CAR-T cell therapy in patients with malignancies were included. Our primary outcome of interest was complete response (defined as the absence of detectable cancer). Two independent reviewers extracted relevant data, assessed risk of bias, and graded the quality of evidence using established methods. A total of 42 hematological malignancy studies and 18 solid tumor studies met were included (913 participants). Of 486 evaluable hematologic patients, 54.4% [95% CI, 42.5%-65.9%] experienced complete response in 27 CD19 CAR-T cell therapy studies. Of 65 evaluable hematologic patients, 24.4% [95% CI, 9.4%-50.3%] experienced complete response in seven non-CD19 CAR-T cell therapy studies. Cytokine release syndrome was experienced by 55.3% [95% CI, 40.3%-69.4%] of patients and neurotoxicity 37.2% [95% CI, 28.6%-46.8%] of patients with hematologic malignancies. Of 86 evaluable solid tumor patients, 4.1% [95% CI, 1.6%-10.6%] experienced complete response in eight CAR-T cell therapy studies. Limitations include heterogeneity of study populations, as well as high risk of bias of included studies. There was a strong signal for efficacy of CAR-T cell therapy in patients with CD19+ hematologic malignancies and no overall signal in solid tumor trials published to date.
