A prevalent and culturable microbiota links ecological balance to clinical stability of the human lung after transplantation

Abstract: There is accumulating evidence that the lower airway microbiota impacts lung health. However, the link between microbial community composition and lung homeostasis remains elusive. We combine amplicon sequencing and bacterial culturing to characterize the viable bacterial community in 234 longitudinal bronchoalveolar lavage samples from 64 lung transplant recipients and establish links to viral loads, host gene expression, lung function, and transplant health. We find that the lung microbiota post-transplant c… Show more

“…Recently, a potential link between the microbial community composition and lung homeostasis was examined by analyzing bronchoalveolar lavage samples from a longitudinally followed post-transplant study population ( Das et al, 2021 ). Although the lung microbiota proved to be highly variable, there were a few bacterial taxa with high prevalence and/or abundance, and the majority of them were either obligate or facultative anaerobes.…”

“…Among the most prevalent species were P. melaninogenica , Veillonella atypica , Veillonella dispar , Streptococcus mitis , and Granulicatella adiacens (all typical recoveries from the oral cavity). The microbiota was categorized into four distinct compositional states (pneumotypes) where the balanced pneumotype represented a diverse bacterial community, resembling that in the oropharynx and including P. melaninogenica , which occurred in 97.4% of the samples ( Das et al, 2021 ). In addition, this pneumotype had a high immune-modulatory activity and preserved lung stability.…”

“…This could also explain the clustering between pathogenic viruses and GTVs. While GTVs were found frequently in nasopharyngeal secretions of children with acute respiratory disease and also in the context of lung transplantation [ 49 ], it is supposed that they are innocent bystanders more than pathogenic agents [ 50 ].…”

“…Studies have revealed that microorganisms of the genus Sporosarcina can usually be found in bronchial biopsies of children with cystic fibrosis 35 . The Nicod team also reported that Porphyromonas, Rothia, Streptococcus , Neisseria, and other microorganisms are the principal constitutive members in the lungs of healthy individuals 36 . It can be seen that our research results are basically consistent with existing reports that indicate that RS-5645 not only alleviates the organ damage caused by inflammatory-factor storms, but also regulates the microbial flora of the lungs, allows pathogenic flora to revert to normalized flora, and promotes organ repair and micro-ecologic balance.…”
