Extrusion of intracanal debris as well as irrigants is a common occurrence during root canal treatment, and no instrument or technique has thoroughly solved this problem. Because flare-ups may arise with any irritation directed towards periapical tissues, a shaping or irrigation technique should minimize the risk of apical extrusion, even though it may not be prevented. There has been a rapid evolution of root canal instruments and irrigation systems through the last decade, and many have been assessed for their debris extrusion potential. The purpose of this review was to identify publications regarding the evaluation of debris, bacteria and irrigant extrusion during root canal treatment. A PubMed, Ovid and MEDLINE search was conducted using the keywords "apical extrusion", "debris extrusion" and "endodontic treatment". The literature search extended over a period of more than 30 years up to 2012. Content of the review was limited to apical extrusion of debris and irrigants, extrusion of liquid by irrigation methods and bacterial extrusion. Issues relevant to apical extrusion were obtained by further search in the reference sections of the retrieved articles. The review provides an update on the current status of apical extrusion.

In this study, 120 new K-files and Hedström files from 3 different manufacturers were examined for surface debris with the scanning electron microscope. The instruments were examined immediately upon removal from their original packages. A second examination of the instruments was performed after they were cleansed in an ultrasonic bath. None of the studied instruments were free of metallic spurs or foreign particles as received from the manufacturers. The ultrasonic bath used together with a cleanser solution appeared to be an effective method to minimize the surface impurities from the manufacturing process prior to use in patients.

The purpose of this study was to investigate the quantity of debris which was extruded apically after canal instrumentation using different types of enlarging instrument in endodontic resin models.Five groups of 9 endodontic resin models were instrumented using each different technique : hand instrumentation without early coronal flaring, hand instrumentation after early coronal flaring, and three nickeltitanium engine-driven instrumentations (Hero 642, Protaper, K 3 ). Debris extruded from apical foramen during instrumentation was collected on preweighed CBC bottle, desiccated and weighted using electronic balance. The results were analyzed using Kruskal-wallis test and Mann-Whitney U rank sum test at a significance level of 0.05.The results were as follows: 1. All of instrumentation techniques produced apically extruded debris. 2. Group without early coronal flaring extruded significant more debris than groups with early coronal flaring. 3. There was no significant difference among early coronal flaring groups. The early coronal flaring is very important to reduce the amount of debris extruded apically.
