A Predictive Model of the Effects of Depression, Anxiety, Stress, 6-Minute-Walk Distance, and Social Support on Health-Related Quality of Life in an Adult Pulmonary Hypertension Population

Abstract: The findings indicate depression, anxiety, stress, 6-minute walk distance, and social support play important roles in HRQOL among patients with PH. Advanced practice nursing strategies to increase HRQOL include counseling, psychiatric referrals, psychotherapy, guided imagery, leading support groups, and low-grade resistance training.

“…One might speculate that as men reported a larger social support and network and more often living with a partner, this might have helped them to access and comprehend information. In addition, women having less social backing is in concordance with a previous study where a diagnosis of PAH, which is a predominantly female disease, caused a reduction in social support [23]. Thus, it is vital that healthcare professionals make an effort to support patients, in the discussions on positive coping strategies include the importance of involving the next of kin in what it means to live with a lifelong disease.…”

“…This is in line with other studies that found an association of low NYHA class with better HRQoL and high NYHA class with worse HRQoL in PH patients [35,36]. In line with others, we could confirm a correlation between HRQoL and exercise performance [4,5,6]. …”

“…Cognitive deficits were found in more than half of PAH patients and mainly presented as worsening in verbal learning, delayed verbal memory, reduced executive function and failed fine motor scores. A decline in cognitive function might also be associated with reduced health-related quality of life (HRQoL), which itself is known to be associated with disease progression [3,4,5,6]. …”

“…The differential impact of drug therapy, health provider support or patients' own coping strategies cannot be defined according to our study, but would be helpful and important to know for health care providers in order to offer the best strategy to support their patients in achieving not only better functional capacity and exercise performance, but also less mood disorders. Our data support a transient improvement of mental disorders by modern PH treatment along with improvements in functional class and HRQoL [29,30,31,32]. In an ethnographic study by Kingman et al published in 2014 [33], PH patients were divided into solution seekers and disease-dominated.…”
