A Practical Approach to Using Adjuvant Analgesics in Older Adults

Abstract: The adjuvant analgesics are a large and diverse group of drugs that were developed for primary indications other than pain and are potentially useful analgesics for one or more painful conditions. The "adjuvant" designation reflects their early use as opioid co-analgesics for cancer pain. During the past 3 decades, their role in pain management has changed with the advent of many new entities, emerging data from numerous analgesic trials, and growing clinical experience. Many of these drugs are now used as pri… Show more

“…Al-Qurain et al (2020) attributed polypharmacy, multiple comorbidities, and altered physiology as factors that increase the vulnerability of adults aged 65 and older to adverse effects of analgesics. From a quality of life perspective, pain is a common cause of symptom distress and functional impairment in adults aged 65 and older, requiring judicious drug selection to ensure safe therapy given the varied adverse-effect profiles of analgesics and potential interactions with multiple other medications (Portenoy, 2020).…”

“…Glucocorticoids may be helpful for selected patients as well as other adjuncts, like antidepressants and antiepileptics such as gabapentin [21,22]. Actually, for patients with neurologic deficits or pain associated with spinal cord compression, high-dose glucocorticoid therapy is part of the standard treatment -a typical dose is 10 mg dexamethasone intravenously followed by 16 mg daily orally in divided doses, until definite treatment [23].…”

“…45 Tricyclic antidepressants are the most widely studied drug therapy for neuropathic pain, with proven evidence of benefit, 46 although they also can be used for chronic musculoskeletal pain. 45 Adverse effects in older adults include anticholinergic and cardiac side effects, and interactions with other medications. 29 Amitriptyline should be avoided in older adults due to its harmful effects.…”
