COVID-19 vaccinations began globally at the end of 2020. By the end of 2021, 9.8 million doses were given in Finland. Regarding safety, most vaccine-related adverse reactions have been mild, but serious and lethal ones have also occurred. Autopsies in post vaccination deaths may give insight to the extent of fatal health conditions with potential COVID-19 vaccine etiology and provide new hypotheses of possible causalities between vaccination and severe health conditions. We searched the complete documentation on all medicolegal autopsies in Finland between December 2020 and December 2021 to assess how often the basis for autopsy was a suspected fatal adverse reaction to COVID-19 vaccination, and whether vaccination remained a potential etiology for any health condition determined as a cause of death after the autopsy. We linked register-based data on individual COVID-19 vaccination course and pre-existing health conditions. We found 428 autopsy cases with a mention of COVID-19 vaccination, and prior to autopsy, vaccination was suspected to play a part in 76 deaths. Post autopsy, a forensic pathologist considered vaccination as a potential etiology in five underlying and seven contributory causes of death. These included seven thromboembolisms, two diabetic ketoacidoses, one myocarditis, one acute pancreatitis, and one eosinophilic granulomatosis with polyangiitis. In relation to the number of vaccinations within Finland, a suspicion of vaccine-related serious adverse reaction was rarely an indication for medicolegal autopsy. Even less frequently was vaccination considered to play a part in the process leading to death, although considerable doubt remains in the accuracy of individual considerations, and autopsy cannot definitively confirm causality between vaccination and death. Regarding vaccination safety, continuing evaluation of suspected vaccine-related deaths is essential, and an autopsy should be part of the investigation when such a suspicion arises.

BackgroundWe conducted an overview to assess immune adverse effects associated with the COVID-19 vaccine, guiding safer choices and providing evidence-based information to clinicians.MethodsForty-three studies on adverse effects of vaccines were reviewed from PubMed, Embase, and Web of Science. Single-arm meta-analyses estimated summary effects, incidence, presentation, etc. An overview using single-arm meta-analysis and reported the findings following the guidelines outlined in the ‘Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) specifically focusing on myocarditis and thrombosis. After screening 2,591 articles, 42 studies met the inclusion criteria. Methodological quality was evaluated using AMSTAR 2. Disagreements were resolved via consensus. Data analysis utilized a random-effects model in R software to estimate incidence rates of selected adverse events.ResultsAfter removing 1,198 duplicates and screening out irrelevant articles from a total of 2,591, we included 42 studies. Adverse reactions to vaccinations include myocarditis, thrombosis, skin reactions, GBS, etc. thrombosis and myocarditis are the most dangerous diseases associated with vaccination. Myocarditis occurred in 6% of Vector vaccine recipients, compared to 61% of mRNA vaccine recipients. Thrombosis was more common after Vector vaccination (91%) than after mRNA vaccination (9%). Furthermore, eight studies conducted anti-PF4 antibody tests and yielded a positivity rate of 67%. Meta-analysis showed that among all patients with Vaccine-induced Thrombotic Thrombocytopenia, cerebral venous sinus thrombosis occurred in 66%, and intracranial hemorrhage occurred in 43%. The rates of deep vein thrombosis and pulmonary thromboembolism in vaccinated patients were 13% and 23%, respectively, with a pooled case fatality rate of 30%.ConclusionThe results of this overview indicate the majority of adverse reactions are self-limiting and require minimal intervention, while rare occurrences such as myocarditis and thrombosis pose a potentially fatal threat.

Introduction:This study investigated the clinical and radiological characteristics and demographics of patients who developed pulmonary embolism (PE) after Coronavirus disease-2019 (COVID-19) vaccination. Methods: The cases of PE were analyzed retrospectively. Data on clinical, demographic, and radiologic characteristics, laboratory findings, Pulmonary Embolism Severity Index (PESI) scores, early mortality scores, PE severity classes, and risk categories for early mortality (low-risk, intermediate-high-risk, intermediate-low-risk and high-risk) were collected from patient files as defined in the European Society of Cardiology guidelines. Patients were divided into two groups: those who had received a COVID-19 vaccine (group 1) and those who had not (group 2). Patients who developed PE within 1 month after vaccination were analyzed separately. Results: A total of 97 patients were included in the study, of whom 61 (62.9%) patients were female. Seventy-five (77.3%) study patients with PE had a history of COVID-19 vaccination (group 1), and 22 (22.7%) had never been vaccinated (group 2). Five (6.6%) patients had received a vaccine within 1 month before PE developed. No significant differences were found between groups 1 and 2 regarding demographics, clinical and radiologic characteristics, laboratory findings, PESI scores, early mortality scores, and PE severity classes (p>0.05), except for mean pulmonary artery pressures. The mean age of patients who developed PE within 1 month after vaccination was 74.6 years, and 80% of these patients were female. The average time to PE after vaccination was 22.2 days, and the mean PESI score was 86.6. Two patients (40%) were in the low-risk category for early mortality and one patient (20%) was in the intermediate/high-risk category. Conclusion: Overall, the characteristics of patients who developed PE after COVID-19 vaccination were comparable to those of patients who had not received a vaccine injection. No statistically significant increases in the incidence of PE were observed within the first month after being vaccinated.
