A population-based study of the epidemiology and clinical features of methicillin-resistant Staphylococcus aureus infection in Pennsylvania, 2001–2010

Abstract: SUMMARY No U.S. general population-based study has characterized the epidemiology and risk factors, including skin and soft tissue infection (SSTI), for healthcare-associated (HA) and community-associated (CA) methicillin-resistant Staphylococcus aureus (MRSA). We estimated the incidence of HA and CA-MRSA and SSTI over a 9-year period using electronic health record data from Geisinger Clinic in Pennsylvania. MRSA cases were frequency matched to SSTI cases and controls in a nested case-control analysis. Logisti… Show more

“…Several risk factors have been reported for MRSA SSTIs, including younger age, exposure to healthcare workers, previous antibiotic use, chronic skin conditions, close contact with a person with MRSA or with a skin condition [5][6][7][8][9]. Additionally, nasal carriage of MRSA has been cited as a risk factor for the development of systemic infections and represents a common reservoir during SSTI outbreaks [10].…”

“…Several national and local studies have documented a decrease in the incidence of invasive MRSA infections in the United States (2)(3)(4). However, MRSA is still estimated to cause ϳ80,000 invasive infections and result in 11,000 deaths annually in the United States (5).…”

“…These children represent the general population in the region. 11,12 The study area included 37 counties in central and northeastern Pennsylvania and consisted of 1288 communities (defined as townships, boroughs, or census tracts in cities 11,13,14 ). Only children whose home address could be geocoded to the street level were included in the analysis.…”

“…Of these, 206 395 had at least 1 height and weight measured in the clinical setting, after deleting biologically implausible values. A total of 173 048 children were successfully geocoded as previously reported 13,14 and 163 820 were included in the analysis after exclusion of 2-year-olds because of difficulties in measuring height at this age. Data were obtained on sociodemographics, encounters, vital signs, laboratory tests, procedures, and medications.…”
